Objectives: The Centers for Medicare & Medicaid Services (CMS) is making available planning grants to state Medicaid agencies to support the implementation of a new state plan option, community-based mobile crisis intervention services for Medicaid recipients in the community who are experiencing a mental health or substance use disorder (MH/SUD) crisis. The purpose of this notice of funding opportunity (NOFO) is to solicit applications for state planning grants to provide funding for developing state plan amendments (SPA), section 1115 demonstrations, section 1915(b) or 1915(c) waiver program requests (or amendments to existing waivers) to provide qualifying community-based mobile crisis intervention services under the Medicaid program. Eligible applicants are state Medicaid agencies.. • Support Alignment Networks (SANs) formed by group practices, health care systems, and others that join together to serve as trusted partners to provide clinician practices with quality improvement (QI) expertise, best practices, coaching and help as they prepare and begin clinical and operational practice transformation. These organizations will collaboratively lead clinicians and practices through the TCPI phases of transformation, achieve the TCPI goals, and through adaptive redesign position these clinicians and their practices to be sustainable components of the changing care delivery system.
