For the last 35 years, the AIDS/HIV Program at the Wisconsin Department of Health Services has coordinated the state’s public health response to the HIV epidemic, with an emphasis on collaboration and coordination between various disciplines in public health. Between 2007 and 2016, both the number and the rate of new HIV diagnoses in Wisconsin declined, with the number of new diagnoses over the last decade going from a high of 283 (2009) to a low of 221 (2014 and 2016), with an average of 244 new diagnoses per year. However, in 2016, 65% of new diagnoses in the state were among racial/ethnic minorities even though these groups make up only 17% of Wisconsin's population. Men who have sex with (MSM) men bear a disproportionate burden of the disease. In 2016, 80% of new diagnoses were attributed to male-male sexual contact. Within these populations, gay and bisexual men of color are disproportionately impacted. In 2016, new diagnoses increased in men under age 30, particularly among Black men whose transmission risk was male-male sexual contact. To reduce new HIV infections, during the grant period supported by PS18-1802 the Program will implement HIV surveillance and prevention interventions consistent with the National AIDS/HIV Strategy and Wisconsin’s Integrated HIV Prevention and Care Plan. Supported activities will include:  utilizing HIV surveillance data to focus prevention efforts and target disparities;  providing access to HIV testing for persons at increased risk (MSM, PWID, partners of known HIV+ persons);  effective referral of newly-diagnosed HIV-positive persons to support engagement and retention in care, and sustained viral suppression;  promotion of PrEP (Pre-exposure Prophylaxis) in HIV negative persons at risk;  increased STI screening among MSM, including extra-genital screening and routine screening for HIV-positive persons;  molecular HIV surveillance, and other strategies to identify and quickly address clusters and outbreaks of HIV and STI, and coordinating state-wide HIV partner services activities with STI DIS efforts;  growing the Data to Care Program in order to re-link individuals to HIV care;  promotion of effective and consistent condom use by HIV-positive individuals, persons with STDs, MSM and other high-risk populations;  supporting behavioral, social marketing and health education programs that promote the reduction of HIV risk behaviors; and  coordination of community planning efforts to engage populations, faith communities and businesses in local HIV prevention efforts. Collaboration within public health and with outside partners will continue to be a priority for the AIDS/HIV Program in 2018. HIV has been reportable in Wisconsin for over 30 years, and the Program's Surveillance systems are mature and well established. Surveillance and HIV Partner Service teams communicate directly with each other. The Program has coordinated a joint HIV Prevention and HIV Care Community Planning process since 2007. In recent years, the Program has played an active role in advising statewide policy and procedures to address the opioid epidemic in Wisconsin, including supporting overdose prevention interventions. The AIDS/HIV Program works closely with the Wisconsin STD Program, jointly funding Milwaukee's DIS activities. The STD Program prevention staff is supervised by the HIV Prevention Supervisor and the state's Adult Hepatitis program is housed within the AIDS/HIV Program and supervised by the Program’s Director. The Program has submitted an application for funding under Component B - Demonstration Projects, which will support additional high-intensity HIV prevention efforts in Milwaukee specifically targeting young Black MSM, focusing on building capacity among LGBT persons of color, specifically young Black MSM, to serve in leadership and decision-making positions in HIV service and social service agencies serving their peers in Milwaukee, Wisconsin.
