NJ Div. of Medical Assist. & Health Svcs. plans to provide a set of targeted pre- and post-release Medicaid services at least 30-days pre- and post-release for juveniles in carceral settings. - Project Abstract: New Jersey Pathway to Reintegration The New Jersey Division of Medical Assistance and Health Services (DMAHS) is requesting funding under NOFO Opportunity number CMS-2T2-25-001, to provide a set of targeted pre- and post-release Medicaid services at least 30-days pre- and post-release for juveniles in participating correctional settings. The funding will support DMAHS in designing the programs, preparing facilities and providers for new services, and developing necessary IT systems. The goal of this project is to prepare DMAHS, facilities, and providers to provide and document the following current NJ FamilyCare services to eligible incarcerated youth and adults reentering their communities: Targeted Case Management (TCM); Early and Periodic Screening, Diagnostic, and Treatment (EPSDT); Physical and Behavioral Health Clinical Consultation Services intended to support the creation of a comprehensive, robust, and successful reentry plan, such as clinical screenings and pre-release consultations with community-based providers; New Jersey will phase in implementation of this initiative to participating facilities based on readiness to implement services. When implementation is complete, this initiative will result in the availability of those services during the implementation readiness phase for Medicaid-eligible populations in all state prisons, county jails and youth correctional facilities. The NJ DMAHS is requesting $1.3 million to support the goal of developing operational capabilities, including implementation readiness, facility and provider readiness, systems development, stakeholder engagement, and monitoring capabilities to promote continuity of care for Medicaid and Children’s Health Insurance Program (CHIP) eligible individuals who are inmates of a public institution. NJ DMAHS plans to be the sole recipient of this funding, with the plan to partner with other organizations critical to the goals of the program, such as Community Based Organizations (CBOs), such as Oaks Integrated Care, which offers mental health, addiction and developmental disability services to children, adults and families.
