Capitol Ave., Pierre, SD 57501 Contact Phone#: 605-394-2495 Contact Email: carrie.churchill@state.sd.us Website: www.doh.sd.gov Purpose: The South Dakota Department of Health seeks to improve pregnancy outcomes, child health and development and early childhood systems by implementing the Nurse Family Partnership and Parents as Teachers evidence-based home visiting models in the following rural, at-risk counties: Beadle, Bennett, Butte, Day, Fall River, Jackson, Lawrence, Marshall, Oglala Lakota, Roberts. Expansion to the following rural, at-risk counties is proposed: Todd and Hughes. Communities were identified based on capacity, fit of early childhood home visiting with current initiatives, and readiness for such services. Goals and Objectives: Goal 1: Increase the capacity of SD MIECHV programs to implement effective evidence-based home visiting services. Objective 1.1: By September 2024, increase the number of at-risk counties from 3 to 5 that are serving priority populations most in need of home visiting services using the PAT model. Objective 1.2: By September 2024, maintain at least 80% of expected MIECHV caseload enrollment in sites that have been operational for at least one year. Objective 1.3: By September 2025, enhance continuous quality improvement efforts to include family engagement efforts representing priority populations served to improve home visiting services. Goal 2: Advance Early Childhood Comprehensive System Development in identified at-risk communities by September 29, 2025. Objective 2.1: Maintain active partnership in the SD Early Childhood Comprehensive Systems grant team, led by the Center for the Prevention of Child Maltreatment, to enhance family engagement and system coordination efforts through September 2025. Objective 2.2: By September 2024, identify existing or develop new local Community Advisory Boards in communities identified for service delivery to ensure that diverse perspectives are represented in discussions of fit and feasibility of maternal and early childhood home visiting services. Methodology: • The South Dakota Home Visiting Program implements the Nurse Family Partnership in combination with SDDOH employees and subrecipient local implementing agencies (LIAs) and partners with a subrecipient LIA to implement the Parents as Teachers evidence-based home visiting model. • Communities served were identified in the 2020 Statewide Home Visiting Needs Assessment (Revised), after assessing number of eligible families and community capacity to support home visiting. • The current caseload of MIECHV family slots (10/1/2022-9/30/2023) is 183. The proposed caseload of MIECHV family slots for (10/1/2023-9/30/2024) is 233. The proposed caseload of MIECHV family slots for (10/1/2024-9/30/2025) is 233. • The DOH works with state agency partners, private service providers, and tribal entities to coordinate services for young families in the state. SD Home Visiting is and active partner in the state’s ECCS. Home visiting staff network at the local level to maintain a referral base, program growth, and coordination.
