This FY23 $4,226,953 award will be used to staff and implement the Parents as Teachers (PAT)® model with fidelity to serve up to 540 families by Sept. 29, 2025. Goals & Objectives: The following goals and objectives for this program period include: 1 Goal: MSDH will establish the MIECHV-MS Program to deliver evidence-based home visiting using the Parents As Teachers (PAT) model. • 1.1 Objective: By Dec 31, 2023, MSDH will become a PAT Affiliate by submitting an Affiliate Plan detailing plans for affiliate design, infrastructure, supervision and professional development, recruitment and retention of families, PAT services to be offered, and evaluation and continuous quality improvement effort • 1.2 Objective: By March 31, 2024, MSDH will hire personnel to implement the PAT model in 16 counties, including a MIECHV Program Director, Parent Educator Home Visitors, and Parent Educator Supervisors • 1.3 Objective: By March 31, 2024, the MIECHV-MS Program will develop and implement policies and procedures as required by PAT. • 1.4 Objective: By June 30, 2024, all MIECHV-MS Parent Educators and Supervisors will complete required PAT foundational and model implementation training • 1.5 Objective: Through Sept 29, 2025, collect and report all required data and program activities to HRSA and PAT 2 Goal: The MIECHV-MS will provide PAT, an evidence-based home visiting model, to eligible pregnant women and children up to kindergarten. • 2.1 Objective: By Sept 29, 2024, MSDH will recruit, enroll, and provide services for 360 families, consisting of pregnant women and/or families with children up to five (5) years of age, across 16 counties in the MIECHV-MS PAT Program. • 2.2 Objective: By Sept 29, 2025, MSDH will recruit, enroll, and provide services for 540 families, consisting of pregnant women and/or families with children up to five (5) years of age, across 16 counties in the MIECHV-MS PAT Program 3 Goal: The MIECHV-MS will engage in continuous quality improvement to implement the PAT model with fidelity by meeting or exceeding the Essential Requirements. • 3.1 Objective: By Jan 31, 2024, the MIECHV-MS PAT Program will establish an Advisory Committee and maintain partnership agreements through the duration of the performance period. • 3.2 Objective: By Sept 29, 2024, the MIECHV-MS PAT Program will establish a continuous quality improvement (CQI) team to implement changes to improve the program procedures, practices, and workflow using Plan-Do-Study-Act (PDSA) methodology. Methodology: The MSDH will implement the PAT evidence-based home visiting model in DeSoto, Tunica, Coahoma, Tallahatchie, Sunflower, Washington, Humphreys, Holmes, Sharkey, Issaquena, Neshoba, Hinds, Claiborne, Copiah, Jefferson, and Wilkinson Counties. Families with pregnant women and/or children up to 5 years of age prior to kindergarten entry who have one or more risk factors for poor maternal and/or child health outcomes will be eligible for enrollment. The MSDH was first awarded MIECHV funding for FY22 with a project period to begin 4/1/2023, to serve 96 families by 9/29/2023. During year 1, the MSDH proposes to serve a maximum of 360 families. During year 2, the MSDH proposes to expand to a maximum of 540 families.
