Activities include providing technical assistance to local implementing agencies, data collection and reporting, continuous quality improvement (CQI), and coordination of comprehensive services to at-risk families. GOALS: Goal 1: To strengthen and improve the state’s infrastructure, activities, and programs carried out under Title V. Goal 2: To improve coordination of services for at-risk communities. Goal 3: Identify and provide comprehensive home visiting services to improve outcomes for eligible families who reside in at risk communities and continually monitor service delivery. METHODOLOGY: Minnesota’s will support Nurse Family Partnership, Parents as Teachers, and Maternal Early Childhood Sustained Home visiting. Proposed Caseload of Family Slots: 998 (FFY24); 1178 (Estimate FFY25); Current Caseload: 998 Key Activities: MDH-FHV will provide oversight and monitoring of budget and expenditures, technical assistance, custom reports, and feedback on CQI improvements and activities to each LIA. MDH-FHV will also work with Title V and other state agencies to implement and maintain the support for EBHV best practices through enhanced linkages with other maternal and child health program planning. Providing data reports to monitor fidelity, progress towards meeting MIECHV benchmarks, and identify areas for technical assistance and CQI will be a strong focus.
