The District of Columbia Department of Health (DC Health) will provide high quality, evidence-based home visiting services to at least 170 eligible families of prenatal women and children ages zero-five, living in very low-income communities through the implementation of two (2) evidence-based home visiting models with fidelity: Parents as Teachers (PAT) and Healthy Families America (HFA). These services will target Health Planning Groups (formerly called Neighborhood Clusters) located within Wards 4,5,7 and 8 which have been identified geographical areas with the lowest socio-economic indicators and poorest health outcomes in the District of Columbia. These Health Planning Groups are: 2,8,16,17,18,19,21,23,28,29,30,31,32,33,34,35,36,37,38, and 39. This project will aim to strengthen and support families through phases of COVID 19 response and recovery, identify areas of improvement for health equity, address staff well-being and retention, and target strategies to improve program sustainability. Through surveys and focus groups, ongoing staff training, and continuous quality improvement, DC Health will aim to attain these goals. Project staff will continue to coordinate and integrate voluntary early childhood home visiting services with sister agencies, particularly the Department of Human Services, DC Child and Family Services Agency, and the Office of the State Superintendent of Education.
