Yellowhawk Tribal Health Center Native Public Health Resilience Grant - Yellowhawk Tribal Health Center (Yellowhawk) is based on the Confederated Tribes of the Umatilla Indian Reservation and serves nearly 4,000 Tribally enrolled individuals and their descendants. Providing holistic, wraparound care to the community, Yellowhawk was awarded accreditation through the Public Health Accreditation Board in September 2020 and is dedicated to public health prevention, education, and resiliency work. Currently, Yellowhawk’s Public Health Department has 28 employees and provides a range of services from early infant and maternal care to chronic disease prevention to Elder services and everything in between. Through Native Public Health Resilience grant funding, Yellowhawk will expand and strengthen foundational public health services. Funding will be used to add or secure staff, supplies, and training to manage projects such as the implementation of our Home Visiting program, the rebuilding of our Communicable Disease program and data collection, the revision and implementation of our Emergency Operations Plan and Quality Improvement policy, and the evaluation, engagement, and implementation of our 2026 Community Health Assessment and 2024-2027 Community Health Improvement Plan. Our work will be centered on understanding community needs and assets, engaging in quality improvement work across our public health programming, and developing plans and policies informed by our community.
