KCIC engages these providers in ongoing performance improvement initiatives, including management of time-sensitive diagnoses and chronic conditions. Supported by the University of Kansas Health System, the state’s only academic medical center, KCIC’s activities are led by an experienced management team and staff members. This team engages directly with participating organizations, providing on-site education and support relating to all initiatives. KCIC’s governing body is comprised of participant representatives, ensuring full engagement in planning and execution of str
ategy. KCIC has successfully managed CMMI and HRSA grants. KCIC has provided vital clinical and operational support for its participants in responding to COVID-19. KCIC’s centralized team of health coaches has furnished chronic care management services for nearly 7,200 unique rural Medicare beneficiaries. Funding Preference: KCIC is requesting a funding preference based on qualification 1. As documented in Attachment 10, all network members are located in counties designated as HPSAs. KCIC also is requesting special consideration based on the inclusion of a signed MOU with all network partners included in Attachment 11.
