The Colorado SBIRT project will integrate SBIRT within existing state initiatives, expanding and enhancing Colorado's capacity to address the needs of individuals at risk for and living with substance use and co-occurring disorders. Utilizing lessons learned from four years implementing screening, brief intervention, and referral to treatment, the project will further its dissemination to unreached primary care and community settings and establish the model as a standard of care. The project will disseminate SBIRT as a standard of care to primary care clinics and community-based settings serving diverse populations by creating a comprehensive strategic plan to implement and evaluate the expansion of SBIR T services and to ensure fidelity to the model; facilitating implementation of SBIRT and mental health clinical guidelines in 25% of Colorado's safety net health clinics; integrating SBIRT in HIV service agencies and employee assistance programs; incorporating SBIRT in components of state health reform initiatives; and utilize care coordinators to collect SBIRT data in six sites, serving 78,000 people through life of project-15,600 annually.
