Feasibility study of an integrated intervention for social isolation and mental health among older adults living with HIV - PROJECT SUMMARY/ABSTRACT Social isolation and loneliness are serious threats to the health and well-being of older adults and are associated with increased risk for cardiovascular disease, cognitive decline, depression, and premature mortality (priorities of the National Institute on Aging). Life course transitions such as loss of loved ones, declining health, and the onset of disability increase isolation risk in later life. Older adults living with HIV face additional challenges to maintain strong and healthy social connections, including a high burden of comorbid mental health conditions (like depression) that further limit opportunities for social participation and may blunt the effects of existing interventions. This R21 proposes a behavioral intervention development study to pilot VV+, a web-based program integrating a community-informed social engagement platform (Virtual Village) with Problem Management Plus (PM+), a brief mental health intervention developed by WHO. Building on prior stakeholder-driven research, this study will recruit 80 older adults (ages 50+) living with HIV and randomize participants 1:1 to VV+ or a healthy aging control condition for a 10-week intervention with 5-week follow-up. There are two aims: (1) Assess feasibility and acceptability of the VV+ intervention by examining recruitment and retention, engagement, implementation fidelity, and participant feedback and (2) Assess preliminary signals of impact on social engagement, network size, loneliness, depression, anxiety, and overall well-being to inform intervention refinement and future trial design. This project directly addresses the goals of PAR-25-355 Multidisciplinary Studies of HIV/AIDS and Aging (R21 Clinical Trial Optional) by focusing on the behavioral and psychosocial aspects of aging with HIV to improve prevention and management of common comorbidities. Additionally, this proposal is in line with the Office of AIDS Research emphasis on non-infectious conditions or mental health disorders, linked with HIV. Findings will inform a future R01 efficacy trial and contribute to the development of scalable, elder-friendly, technology-based strategies to promote healthy aging for people with HIV.
