Program Sustainability Option:Option 4: Attachment 7 Letter of Commitment Projected Total Number of Residents: 2 per year (6 total during the 3-year residency) Expected ACGME Accreditation and Residency Matriculation Dates: AY27 Funding Priority Points Requested: Funding Priority 2 - Maternal Health Executive Summary: The URMC/TCFM Rural GME Consortium (“Consortium”) is comprised of URMC (sponsoring AMC), Highland Hospital (urban hospital & FM practice), Noyes Health (rural community hospital, ASC and outpatient mental health), and Tri-County Family Medicine Program, Inc (TCFM) (FQHC Look-alike:6 rural practice sites). The Consortium’s purpose is to add to URMC’s Highland Family Medicine Residency an ACGME accredited Rural Track Program (RTP) with Maternal Health & Obstetrics Pathway (MHOP), collectively “HFM RTP with MHOP”, to increase access to Family Medicine and Obstetrics for our target population & all rural U.S. counties. Target population includes minority & low-income rural residents & all women of reproductive age in Allegany, Livingston, Steuben & Wyoming counties of NY. Physicians trained through this RTP will be proficient in the full spectrum of FM and able to independently manage prenatal & postpartum care & perform deliveries for pregnant persons in rural areas. Program goals, objectives, and expected outcomes: URMC/TCFM Rural GME Consortium will collaborate to increase access to healthcare by underserved minority populations & preserve access to family medicine & obstetric care in the target rural service area. During the 3-year planning grant, the Consortium will: 1. Appoint key Residency program staff by 8/1/2024. 2. Establish URMC Subcontract with TCFM for RTP sites by 7/15/2023. 3. Establish the Rural Track Program Curriculum, per ACGME requirements & submit an RTP Rotation Information Form by 7/1/2024 and ACGME application by 8/1/2025, with matriculation of our first 2 residents (PGY-1) by 7/1/2026. 4. Establish RTP Training Sites, by working with the Consortium to identify preceptors & identify & prepare FM, OB, & specialty training sites (Staffing, IT, Operations) with sufficient volume & diversity of cases to fulfill ACGME training requirements & prepare competent providers. Team will ensure a supportive environment (Social programming, Housing) that facilitates resident engagement, as reflected in our surveys. 5. Finalize an RTP Sustainability Plan to be submitted by 8/1/2024. 6. Modify HFMR Graduate Tracking Plan to accommodate this RTP by 6/30/2029.
