Located in Pocatello (83201), the ISU RWPC Program serves a large frontier / rural geographic catchment area consisting of the Eastern and Southeastern Idaho Public Health Districts. This includes the counties of Bannock, Bear Lake, Bonneville, Bingham, Butte, Caribou, Clark, Custer, Franklin, Fremont, Jefferson, Lemhi, Madison, Oneida, Power, and Teton. The ISU RWPC Program is proposing the development of the “HIV Oral, Psychological, and Emotional Support (HOPES) Program”, designed to integrate behavioral health and oral health services more seamlessly with HIV primary care in a sustainable, interprofessional, and trauma informed manner. The HIV community in Eastern and Southeastern Idaho needs capacity development for oral and behavioral health for many reasons that will be expanded upon in this application, but in short: (1) 50% of patients living with HIV (PLWH) in our service area have not been evaluated by an oral health provider in the last year and (2) 50% of PLWH in our service area have a mental health diagnosis, 30% have a positive screen for depression
, and 30% of patients admit to current substance use (i.e. methamphetamine, opioids, alcohol, marijuana…). The proposed activities of the HOPES Program will be to create better interprofessional relationships and integration into primary care so the ISU RWPC Program can do the following: 1. Develop new partnerships with community stakeholders who provide oral/behavioral health services and strengthen existing ones to create better access for PLWH. 2. Enhance oral/behavioral care services by integrating oral/behavioral health specialists into HIV primary care; including huddles, streamlining referrals, and educating oral/behavioral health providers about the special needs for PLWH. 3. Expand behavioral care services by incorporating new in-house psychiatry services and integrative behavioral health services from in-house psychologists. Expand oral health services by leveraging financial resources with clinics that offer services on a sliding fee scale.
