Comprehensive High-Impact HIV Prevention Programs for Young Men of Color Who Have Sex with Men Category A - Family Health Centers of San Diego (FHCSD), San Diego County’s largest Federally Qualified Health Center (FQHC) and provider of HIV outreach, prevention and care, proposes to reduce morbidity, mortality, and related health disparities among young men of color who have sex with men (YMCSM) ages 18-29 years and their partners regardless of age, gender and race/ethnicity through a status-neutral Comprehensive High-Impact HIV Prevention (HIP) Program. FHCSD will partner with Vista Community Clinic (VCC), also an FQHC. As lead agency, FHCSD will develop and implement a HIP Program under Category A of the FOA – HIV prevention services for YMCSM and their partners regardless of age, gender, and race/ethnicity. FHCSD will target Latino, African American, multi-race, and other YMCSM, including those with a history of injection drug use (IDU), in San Diego County’s Central, South Bay, and North Regions. FHCSD and its partners will seek to increase the number of YMCSM and their partners who are tested for HIV and aware of their status, referred to PrEP and nPEP, and effectively linked to and retained in HIV care through a comprehensive marketing and outreach campaign, widespread HIV and STD testing, high-impact behavioral interventions for HIV-positive (HIV+) and high-risk HIV-negative (HRN) individuals, comprehensive navigation services, and referrals to prevention and essential support services (e.g., insurance coverage, mental health services, substance abuse treatment, housing, employment assistance, transportation, etc.). FHCSD and its partner will provide targeted HIV and STD testing and linkage-to-care services for YMCSM and their partners across the county’s Central, South, and North Regions. Newly- and previously diagnosed individuals will receive one-on-one navigation services from a trained health educator to access high-quality HIV medical care, medication adherence services, Partner Services (PS), behavioral interventions including Personalized Cognitive Counseling (PCC), and referrals to a spectrum of prevention and essential support services. Health educators will offer the Anti-Retroviral Treatment and Access Services (ARTAS) linkage-to-care intervention to address client barriers and ensure all HIV+ clients access medical care within 30 days of their diagnosis. HRN persons also will receive navigation services, including linkage to PrEP and/or nPEP, the Personalized Cognitive Counseling (PCC) intervention (as appropriate), and linkage to prevention and essential support services including expanded use of Social Network Strategy (SNS). FHCSD will implement the PROMISE for HIP community-level intervention to reduce HIV-related stigma and build awareness of HIV risk, testing, and PrEP/nPEP among HRN YMCSM. FHCSD and our partners will implement a condom distribution program across the county’s Central, South, and North Regions, and will establish a safe space for YMCSM which will empower clients, reduce stigma, racism, homophobia, and transphobia, and reduce barriers to HIV testing and care. It will be the hub of a peer-led Community Affairs Board and a welcoming space utilized for educational workshops, events, and townhall meetings. FHCSD intends to achieve the following outcomes: Increase in number of YMCSM tested for HIV; Increase in HIV+ YMCSM who receive Partner Services; Increase in HIV+ YMCSM who are linked to medical care; Increase in HIV+ YMCSM who receive prevention and essential support services; Increase HRN YMCSM who are referred to PrEP and/or nPEP; Increase the number of HRN YMCSM who are referred to PCC; Increase in HIV+ and HRN YMCSM who are offered condoms; Increase in HRN YMCSM who are referred to a HIP intervention and/or prevention and essential support services; Increase in the number of PROMISE role model stories; Develop and implement a community social marketing campaign; Provision of a safe space for YMCSM in San Diego County; and Engagement of an active Community Affairs Board.
