Oregon Health Authority HIV Surveillance And Prevention Program - The HIV/STD/TB (HST) program of the Oregon Health Authority is pleased to submit this application for cooperative agreement PS24-0047 to implement high impact, comprehensive HIV prevention and surveillance programming to prevent new HIV infections and improve the health of people with HIV. This application aligns with the CDC's DHP strategic focus areas to bolster community engagement, health equity, syndemic and whole-person approaches to HIV prevention. We aim to increase knowledge of HIV/STI status, reduce transmission, prevent new infections, improve rapid linkage to care and viral suppression and maintain elimination of perinatal transmission through data driven program planning and service delivery. Our activities and strategies align with the Healthy People 2030 focus on HIV, STI and health equity and other national public health strategies and priorities outlined in the National HIV/AIDS Strategy for the US (2022-2025); Ending the HIV Epidemic (EHE), CDC's Data Modernization Initiative, and the NCHHSTP Strategic Plan (2022-2026). We will comply with the CDC Technical Guidance (TG) for HIV Surveillance Programs, CDC Data Security and Confidentiality Guidelines, National HIV Prevention Program Monitoring and Evaluation Guidance for HIV Prevention Programs, and the Cluster Detection and Response (CDR) Guidance. This proposal includes a Table of Contents, Project Narrative, Project Work Plan, and Budget Narrative which outline how our integrated HIV Prevention and Surveillance Program will continue to collect high quality case surveillance data to inform and improve high impact HIV prevention. We describe how our program will address all six PS24-0047 strategies and corresponding activities to achieve overarching project goals: to increase knowledge of HIV/STI status; reduce transmission; and prevent new infections through integrated testing, equitable PrEP and PEP access, access to condoms and harm reduction services, perinatal HIV surveillance & prevention, enhanced partner services, rapid access to care and/or re-engagement in care, viral suppression, as well as through community engagement and planning, meaningful collaboration, and cluster detection and outbreak management. We will partner with Oregon’s CDC Project Officer’s during the first 6 months to finalize the EPMP. We have a proven track record of providing high quality, high impact HIV prevention and surveillance programming, and welcome the opportunity to continue the critical work of improving the health of all Oregonians through this award.
