High-Impact HIV Prevention and Surveillance Programs for Health Departments: Alaska - The purpose of the activities outlined in this application are to prevent and reduce new HIV infections; improve health outcomes for persons living with HIV infections; reduce health-related disparities; and to integrate partner collaboration across the HIV prevention and care continuums. These goals are achieved by maintaining a robust HIV surveillance system, ensuring that every person living with HIV is aware of their infection, linking and retaining persons living with HIV into medical care, and by supporting highly impactful community-level HIV prevention activities. Effective monitoring and evaluation strategies will allow program staff to continue to develop and refine supported activities through the project period. Long-term outcomes of this project include: Reducing new HIV infections in Alaksa; Improved health outcomes for Alaskans living with HIV, including sustained viral suppression; and Reduced HIV-related health disparities in Alaska.
