High-Impact HIV Prevention and Surveillance Programs for Health Departments - Title: A comprehensive approach to prevent new HIV infections and improve the health of people with HIV in Ohio. Purpose: Establish, cultivate, and maintain collaborative strategic partnerships to implement comprehensive interventions and public health strategies to prevent new HIV infections and improve the health of people with HIV. Objectives: 1) Preventing new HIV infections, 2) Improving HIV-related health outcomes of people with HIV, 3) Reducing HIV-related disparities and health inequities, and 4) Achieving integrated, coordinated efforts that address the HIV epidemic across the continuum of care. Methods: Implementation, integration, evaluation, and sustainment of comprehensive and scientifically proven, cost-effective, and scalable structural, behavioral, and biomedical interventions; Collection and dissemination of timely and accurate HIV surveillance and program data. Outcomes: 1) Reduced new HIV infections, and 2) Sustained viral suppression among persons with HIV.
