There is a need to strengthen existing agreements with different service providers and develop new re-engagement to care strategies that enable participants to overcome the barriers. - The STD/HIV/TB/Viral Hepatitis Prevention Division (DPEVHV) will implement a Community Health Worker (CHW)-mediated model to facilitate re-engagement of people with HIV (PWH who are not in care and outreach to those who are not virally suppressed in rural communities and/or municipalities. A collaboration network will be established between service providers and clinics, through a collaborative agreement with the Puerto Rico Public Health Trust (PRPHT) to enable access to and availability of prevention and treatment services. There will be disease intervention specialist (DIS) and CHWs who will be assigned and trained to provide services, such as anti-retroviral treatment (ART) delivery, sample collection for laboratory testing, arranging and scheduling telehealth visits, and/or in-person visits with an HIV medical provider and other providers (mental health, primary care), and offer evidence-based medication adherence support.
