The California Department of Health (CDPH) Office of AIDS (OA) is proposing a set of strategies and activities that address the four pillars outlined in the Federal Ending the HIV Epidemic (EtHE) in America initiative. CDPH OA is submitting the grant on behalf of the six Phase I counties (Alameda, Orange, Riverside, Sacramento, San Bernardino, San Diego) assigned to CDPH OA, referred to as the California Consortium. Between 2011 and 2018, four of the six counties saw a reduction in new HIV diagnosis, ranging from five percent less to 48 percent fewer new diagnosis. Two of the counties had noticeable increases in new diagnosis, increasing 25 percent and 26 percent. This application includes strategies and activities to achieve the 75 percent reduction in new diagnosis by the end of 2025. Activities will be directed to Black/African American and Latinx Men who have Sex with Men (MSM), MSM under the age of 30, especially among young MSM of color, transgender women, people using substances—especially methamphetamine and injection drug use, homeless individuals, sex workers, pre-exposure prophylaxis (PrEP) eligible men and women, and individuals without health insurance or not utilizing healthcare services. Strategies include more focused HIV-testing, including taking HIV testing into homeless camps and locations where homeless people seek services routinely, including STD and HCV testing with HIV screening to increase likelihood of People who inject drugs seeking screening, delivery of PrEP or ART at testing sites, and providing essential healthcare such as blood pressure checks, diabetes monitoring, and wound care along with screening. To increase rapid ART, proposed activities include use of peer navigators, same-day HIV appointments for newly diagnosed individuals, increased clinical detailing, social media promoting the benefits of same-day ART initiation, and monitoring the time between diagnosis and confirmation that individuals are on ART. To increase PrEP and PEP usage, activities to be initiated include distributing PrEP in the community through mobile healthcare, peer delivery, promotion of PrEP and PEP availability at pharmacies, benefits counselors to assist in enrollment in assistance programs, and marketing accurate information about PrEP, the Ready, Set, PrEP resource, the California PrEP Assistance Program, and marketing the PrEP Assistance Program for people age 12 and older when it is implemented. In addition, investment in the infrastructure of Sexually Transmitted Disease (STD) Clinics creates a critical opportunity for HIV prevention in California. In California in 2018, 63 percent of men who have sex with men (MSM) with primary and secondary (P&S) syphilis cases were HIV-negative; 4 percent were newly diagnosed with HIV. CDPH estimates that 70 percent of MSM gonorrhea cases in 2018 were HIV-negative, with 53 percent of those MSM reporting that they were not on PrEP. STD clinics diagnose a disproportionately high number of these cases and reach those at highest risk for HIV acquisition, and are thus a priority for initiating HIV testing, linkage to care (LTC), re-engagement in care, and initiation of PrEP/non-occupational post-exposure prophylaxis (nPEP). Three EtHE counties have existing STD clinics which will incorporate HIV prevention services into their STD clinics: Orange, Sacramento & San Diego. This proposal has a set of innovative, comprehensive strategies and many activities that involve community participation in their implementation, and sufficient staff to monitor and evaluate the work being done throughout the California Consortium. OA and the counties are ready to implement the plan when funding is awarded.
