The Infectious Disease Prevention and Health Services Bureau (IDPHSB) in the Prevention and Health Promotion Administration (PHPA) is applying for funding from the Centers for Disease Control and Prevention (CDC) under CDC-RFA-PS18-1802, Integrated HIV Surveillance and Prevention Programs for Health Departments. Our application has three parts: Component A – HIV surveillance throughout the state of Maryland. Component A – HIV prevention for all of Maryland except Baltimore City. Component B – a demonstration project for reducing HIV transmission through enhanced use of pharmacy data for individuals prescribed pre-exposure prophylaxis (PrEP) or antiretroviral therapy (ARV) for HIV disease. HIV surveillance is the identification, investigation, and reporting of cases of HIV infection and AIDS disease and the measurement of death among these cases. Information will be collected from health care providers and laboratories following the reporting requirements in Maryland law and the federal guidelines for performing HIV/AIDS surveillance. Active case surveillance will be implemented to ensure complete and accurate reporting. A data system (HIV and AIDS registry) will be maintained to provide information about the HIV/AIDS epidemic in Maryland. Case information, stripped of personal identifiers, will be transmitted to the CDC for inclusion in the national data system. HIV surveillance information will be used to guide HIV prevention funding and activities, to provide information for partner services, linkage to care, re-engagement in care, and other data to care (D2C) activities, and to identify and respond to transmission clusters. Confidentiality of the HIV/AIDS surveillance information will be maintained according to Maryland law and the federal guidelines on confidentiality and data security. This funding impacts all aspects of Maryland’s response to the HIV epidemic including HIV awareness and program planning. Working with local health departments, community-based organizations, and the Department of Public Safety and Correctional Services this funding will allow for the screening over 25,000 individuals for HIV. In addition, we are requesting PCSI funding to include screening for hepatitis C in all non-clinical testing programs. Infrastructure support and direct funding will allow for the development of PrEP programs in 14 jurisdictions across Maryland. Syringe Service Programs are proposed for 10 counties as a risk-reduction strategy for Marylanders injection drug users. The greatest reach of our proposed HIV prevention program comes through our condom distribution, with over 3,500,000 made available throughout the state. For Component B, PHPA is proposing an innovative use of pharmacy claims data to identify individuals who been prescribed ARV for either PrEP or as therapy for HIV disease. Working with the University of Maryland School of Pharmacy, our treatment adherence demonstration project will bring together data from our surveillance, AIDS Drug Assistance Program, Ryan White health services, Center for STI Prevention, Medicaid, and the clinical pharmacists in practice at the University of Maryland Medical Center, Chase Brexton Health Care, and several local pharmacies to identify individuals who have not picked up their prescriptions. Through data-matching, line lists will be developed of individuals out of therapy compliance and each individual will be reviewed by an interdisciplinary team to identify the most appropriate treatment adherence intervention. This innovative collaborative approach will allow MDH to be at the forefront of the developing effective models for increasing treatment adherence and reducing new infections in Maryland. We estimate that in the first year as many as 100 individuals will improve their treatment adherence.
