Illinois Department of Public Health CDC-RFA-PS18-1802 – Integrated HIV Surveillance and Prevention Programs The Illinois Department of Public Health has an integrated HIV surveillance and prevention program. The Department’s Data Security and Confidentiality Guidelines permit the exchange of HIV testing, HIV surveillance, Direct Services and ADAP data between these programs for the purposes of confirming whether a person is a new or existing case, relaying HIV lab test results, confirming eligibility for partner and linkage to care services and offering partner and linkage to care services. Illinois will employ strategies to prevent new infections; improve health outcomes for persons living with HIV infection, including achieving and sustaining viral suppression; and reduce related health disparities in accordance with the national prevention goals, HIV Care Continuum and CDC’s High-Impact HIV Prevention (HIP) approach by using quality, timely, and complete surveillance and program data to guide HIV prevention efforts. Funding for programming addressing each priority will be divided between Illinois Regions using a weighted epidemiologic formula of 67% HIV incidence and 33% HIV prevalence per current address to ensure that hardest hit areas are most intensely funded in proportion to regional disease burden. Through the five year PS18-1802 project, Illinois will strive to reduce risk and new HIV infections, find people who are already infected but don’t know it, and ensure that everyone living with HIV/AIDS enters and stays in high quality care to improve their quality of life and reduce further HIV transmission. Illinois’ approach to addressing PS18-1802 priorities may be briefly summarized as follows. - Individual knowledge of HIV status will be increased through expanded risk-targeted non-clinical HIV testing, expanded clinical routine HIV screening, perinatal HIV testing in both the first and third trimesters, and Partner Services. Risk-based HIV testing will serve prioritized risk/race/ethnicity groups in proportion to their regional HIV incidence to avoid service disparities and ensure that the most intense efforts to diagnosed persons living with HIV (PLWH) unaware of their status occur among highest incidence risk populations. - New HIV infections among HIV-negative persons will be prevented through comprehensive HIV Prevention for HIV-positive persons (described immediately above and below) and through CDC-supported effective behavioral risk reduction interventions and through the biomedical risk reduction interventions of pre- and post-exposure prophylaxis. Individual and Community-level Risk Reduction Activities (RRA) will serve HIV-negative prioritized risk/race/ethnicity groups in proportion to their regional incidence to avoid service disparities and ensure that the most intense efforts to reduce individual, network and population risk occur among highest incidence risk populations. - HIV Transmission from persons living with HIV (PLWH) will be reduced through linkage to, re-engagement in, and retention of PLWH identified through Department-supported HIV testing or through HIV surveillance into HIV treatment, effective medication adherence interventions to support achieving viral suppression, and into CDC-supported effective behavioral risk reduction interventions. - Interventional surveillance will be strengthened to intensify data-to-care activities to support sustained viral suppression to enhance response capacity for HIV transmission cluster investigations and outbreak response efforts. All of Illinois’ activities will conform to the Surveillance Program’s written Security and Confidentiality Policy as required by the Data Security and Confidentiality Guidelines for HIV, Viral Hepatitis, Sexually Transmitted Disease, and Tuberculosis Programs.
