The California Department of Public Health, Center for Infectious Diseases, Office of AIDS (OA) will use this Notice of Funding Opportunity (NOFO) to integrate the work of the HIV Surveillance, Research and Evaluation Branch and the HIV Prevention Branch to meet the goals of Laying a Foundation for Getting to Zero: California’s Integrated HIV Surveillance, Prevention and Care Plan (IP). These goals are to 1) reduce new HIV infections; 2) increase access to care and improve health outcomes for PLWH; 3) reduce HIV-related disparities and health inequities; and 4) achieve a more coordinated response to the HIV epidemic. OA is responsible for HIV prevention and surveillance activities in the Centers for Disease Control and Prevention- (CDC) defined the California Project Area (CPA) which is the 57 of the 61 local health jurisdictions (LHJ) in California that are not part of Los Angeles County or San Francisco County. In this NOFO, OA will fund the 20 LHJs in the CPA with the highest HIV prevalence for HIV prevention activities including routine, opt-out and targeted HIV testing, linkage to HIV care, retention and re-engagement in HIV care, data to care, partner services, and pre- and post-exposure prophylaxis. HIV surveillance activities will be performed in all CPA LHJs. OA anticipates meeting the following CDC-defined outcomes as well as those of the California Integrated Plan: 1. Improved monitoring of trends in HIV infection; 2. Improved completeness, timeliness, and quality of HIV surveillance and prevention program data; 3. Increased ability to describe the geographic distribution of HIV and understand the social determinants of health in relation to HIV and HIV-related health disparities; 4. Increased number of persons who are aware of their HIV status; 5.Increased participation in HIV partner services among persons with diagnosed HIV infection; 6. Improved early identification and investigation of HIV transmission clusters and outbreaks; 7. Improved response to HIV transmission clusters and outbreaks; 8. Improved plan and policies to respond to and contain HIV outbreaks; 9. Increased linkage to and retention in HIV medical care among PLWH; 10. Increased HIV viral load suppression among PLWH; 11. Increased referral of persons eligible for pre-exposure prophylaxis (PrEP); 12. Improved completeness, timeliness, and quality of perinatal HIV surveillance data; 13. Increased availability of condoms among persons living with or at risk for HIV infection; and 14. Increased data security, confidentiality, and sharing. OA is submitting a Component B project entitled Same-Day PrEP. OA’s proposal will offer a competitive Request for Applications to entities in the CPA that have the ability and willingness to offer same-day PrEP to appropriate gay, bisexual and men who have sex with men of color and transgender women. OA will choose two to four entities to provide this service and ongoing to follow up and adherence services to clients. Expected outcomes are increased interest in, usage and long-term adherence to PrEP.
