The New Mexico Department of Health (NMDOH) has been the lead agency for HIV prevention and HIV surveillance in the state for over two decades. The lead entities for these two collaborative areas are the HIV Prevention Program within the Public Health Division (PHD) and the HIV Epidemiology Program within the Epidemiology and Response Bureau (ERD). These programs and divisions partner closely through engaging in community planning, working on developing the HIV Care Continuum, updating state laws and regulations to improve the use of HIV epidemiology data for public health action, and ensuring data security and confidentiality. Component A - HIV Prevention and Surveillance NMDOH proposes to continue implementation of a comprehensive and innovative HIV prevention and surveillance program to fulfill the goals from CDC as stated in the funding announcement PS18-1802. This will include activities to achieved the goals stated in the eleven required strategies. Intended outcomes over the project period are stated for strategies # 1 (Surveillance Prevention Program Monitoring), 2 (HIV Testing and Partner Services), 3 (Plan Response to Clusters and Outbreaks), 4 (Prevention for Persons Living with HIV), 5 (Prevention for HIV-Negative Persons at Risk), 6 (Perinatal Prevention), 7 (Community Level Prevention) and 9 (Structural Strategies). All work will be guided by the New Mexico Integrated Plan for HIV Prevention and Care: 2017 – 2021 which has goals, objectives and strategies that align with the goals of this funding announcement. This plan was completed and submitted to both CDC and HRSA in September 2016. The state's integrated planning body, the New Mexico HIV Community Planning and Action Group (CPAG), continues to work to update the plan, as well as starting to monitor progress on achieving the stated goals, objectives and strategies. Component B - Demonstration Project to Expand the Provision of HIV Pre-Exposure Prophylaxis (PrEP) NMDOH has increased access to and utilization of HIV PrEP through innovative strategies including partnerships with community-based health centers and private medical providers. While effective, this work has missed opportunities to reach the particularly high-risk population of persons who have reportable sexually transmitted disease (STD) infections or recent sexual contacts with persons infected with STD. The proposed Demonstration Project will reach these groups directly by providing PrEP at traditional STD clinics operated by public health. STD clinics operated by NMDOH have found the most newly diagnosed cases of HIV infection in recent years, making them the ideal venue for recruiting. Mid-level clinicians will be recruited to provide PrEP directly. The proposed demonstration project will be unique in showing the effectiveness of direct provision of PrEP in the traditional STD clinic setting. It will be cost-effective by leveraging existing resources of NMDOH including infrastructure at Public Health Offices such as nursing and clerical support; existing lab services funded by the STD program; partnership with excellent internal and external 340B pharmacies; and existing HIV/STD Health Educators who can provide adherence counseling, follow up and data tracking for evaluation purposes.
