Promoting Good Health and Wellness in CTCLUSI (GHWIC) - The proposed Promoting GHWIC Component 1 project will be implemented by the Confederated Tribes of Coos, Lower Umpqua, and Siuslaw Indians, a federally recognized Tribal government, located in Coos Bay, OR with a service area of Coos, Curry, Douglas, Lane, and Lincoln counties. CTCLUSI Tribal leadership are perpetually focused on enhancing the overall well-being of its Tribal citizens by developing culturally based programs and services to reduce chronic diseases to build stronger, resilient individuals and, in turn, a stronger community. The Promoting GHWIC project will improve the prevention and management to reduce the burden of chronic diseases and increase cultural connectedness among its Tribal citizens. This program will be a partnership between CTCLUSI’s Community Health Department, Education Department, Department of Culture and Natural Resources, and the Three Rivers Health Center. The proposed program will consist of following three strategies: • Provide a foundation for chronic disease prevention by implementing community-chosen, traditional AIAN practices that build resilience and strengthen connections to family, culture, and community; • Establish and enhance systems that connect community members to community, clinical, and social services and programs to prevent, manage, and control chronic diseases and their risk factors; and • Promote the implementation of multi-disciplinary team-based care to prevent, manage, and control chronic diseases and to connect patients to community and social service providers to address health-related social and economic needs.
