NARA NW GHWIC 24 - The Native American Rehabilitation Association of the Northwest (NARA NW) is an Urban Indian Organization and Federally Qualified Health Center (FQHC) working with the mission “to provide education, physical and mental health services, and substance abuse treatment to American Indian and Alaska Natives (AI/AN) and other vulnerable populations.” NARA NW is a unique agency in terms of honoring the cultural heritage and world view of Native Americans and delivering services with clinical practices that are culturally sensitive to the needs of AI/AN clients. No one is turned away from services due to an inability to pay for their care. Through the CDC Cultural Approach to Good Health and Wellness in Indian Country (GHWIC) project, NARA NW seeks to address the disproportionately high rates of chronic diseases among AI/AN adults though a culturally responsive, community-drive approach. With over 20 years of experience with chronic disease programming, NARA NW has established multi-disciplinary teams that provide cardiovascular screening, breast and cervical cancer early detection and prevention, colorectal cancer screening, and diabetes treatment and prevention. These programs provide culturally relevant innovative services and activities, which include enrollment in Healthy Behavior Support Services (HBSS), comprehensive “one-stop shop” diabetes clinics, self-monitoring blood pressure devices and trainings, cooking classes, and walking programs. Furthermore, intensive clinical case management services are offered to support all AI/AN people living with chronic diseases to address social determinants of health, improve quality of life and help prevent complications of chronic diseases. The proposed project is designed to prevent, manage, and control chronic disease in AI/AN communities through comprehensive, holistic care built on community engagement. A special focus of the project will be connecting with our clients and local tribes to evaluate our
services and materials and identify opportunities to integrate more culture and traditional practices. The staff resources supported by the requested funding include a Program Manager at 1.0 FTE, a Community Health Worker at 1.0 FTE, an Evaluation Specialist at 0.4 FTE, and a contracted Public Health Consultant Evaluator. The chronic disease teams are an integral part of the NARA NW primary care clinics and work closely with the medical provider teams to provide quality clinical care, health education and intensive case management for patients living with and/or at risk for chronic diseases.
