Pinoleville Pomo Nation Good Health and Wellness in Indian County Project - The Pinoleville Pomo Nation’s (PPN) proposed project, entitled Good Health and Wellness in Indian Country (GHWIC) project, is geared towards serving rural Tribal population within the PPN service area. The service area of the proposed GHWIC project encompasses Lake County, Sonoma and Mendocino County Native Americans. PPN has developed this program using a multi-pronged approach which addresses high rate of obesity, diabetes, high blood pressure and high blood cholesterol through nutrition education and physical activity, prevention and self-management of chronic disease listed above to increased health literacy, cultural practices to improve health, strengthened team-based health care and community clinical linkage. The disparities data indicates that American Indians have lower health status when compared to other Americans. To account for these disparities, PPN Tribal leaders will address the wellness crisis using the following strategies: (1) Tribal Specific Chronic Disease Education; (2) Prevention Initiatives; (3) Walking Support; (4) Strengthen Cultural Connectiveness and Community Wellness Services; and (5) Establish Clinical Linkage. An intergenerational, multifaceted approach will be taken to enhance the cultural appropriateness of outreach, prevention, and educational services. PPN will allocate funding to establish evidence-based and culturally relevant chronic disease prevention and management program to increase referral, awareness and knowledge within the PPN service area. The proposed project will be administered through PPN’s Health Department, and the formation of communities of practice to improve the cultural relevance of programmatic and evaluation activities. The PPN GHWIC project team will include representatives from the Wellness Program; Environmental Department; Head Start and Early Head Start; Vocational Rehabilitation Department; Learning Center; Community Garden and the Methamphetamine and Suicide Prevention Initiative (MSPI). Members of the PPN GHWIC will collaborate to plan and implement community values into every stage of the process. The PPN will collaborate with Consolidated Tribal Health Project (CTHP), Lake County Tribal Health Consortium (LCTHC), New Life Clinic, North Coast Opportunities (NCO), Mendocino County Public Health, Mendocino County Behavioral Health and Recovery Service and the California Rural Indian Health Board (CRIHB), Kashia Band of Pomo Indians and Little River Band of Pomo Indians to build stronger capacity to strengthen clinical linkage and community outreach. PPN will use GHWIC funding to address these disparities by providing access to traditional wellness activities, programs and classes for the community. Partnerships with Consolidated Tribal Health Program, other local tribes and other agencies play a huge role in the success of these programs, we have partnered with CTHP for our Prevent T2 classes which are hosted at CTHP’s wellness building and participants were referred by health care providers at their clinic. We will host a variety of health classes and workshops, exercise classes and groups. Promote the Connection of patients to community and social service providers to address health- related social and economic needs.
