Paul Coverdell National Acute Stroke Program - The Ohio Department of Health (ODH) Bureau of Health Improvement and Wellness is applying for the Centers for Disease Control and Prevention (CDC) cooperative agreement CDC-RFA-DP-24-0060: Paul Coverdell National Acute Stroke Program. Ohio has a rich history of work in heart disease and stroke. Ohio Coverdell has 17 years of programmatic strategies and activities related to the prevention and management of stroke. ODH is currently funded by The National Cardiovascular Health Program (DP23-0004) and by WISEWOMAN: Well-Integrated Screening and Evaluation of Women Across the Nation (DP23-0003) to implement evidence-based strategies to prevent cardiovascular disease and improve care and management for people with cardiac risk factors. In 2022, stroke was the 5th leading cause of death in Ohio, accounting for 7,202 deaths. Ohio ranks as the 7th most populous U.S. state and has the 5th highest rate of stroke mortality among the nation. Ohio’s 2018-2021 crude stroke mortality rate among adults aged 18 and older is 74.7 per 100,000, far exceeding the U.S. crude rate of 62.9 per 100,000 and the NOFO threshold of 55.9 per 100,000. The burden of stroke mortality is disproportionately higher among Ohio’s Black population. Key projects for Year 1 include the following: (1) Expanding the CVH Quality Improvement Project (QIP) under DP23-0004 to include patients who have experienced a stroke and those with undiagnosed or uncontrolled hypertension within target clinical practices in the priority populations of focus at highest risk of stroke. (2) An Acute Care QIP will be established to address patients that have experienced a stroke and aim to use data elements to improve efficiency and quality of care from emergency care to discharge, establish procedures to assess and refer patients for SDoH needs, and link clinical care with community resources.(3) To address the social service and support needs of patients at highest risk of stroke, Ohio Coverdell will leverage work with CliniSync and Unite Us conducted through DP23-0004. (4) The Ohio Coverdell Program Administrator will join the Ohio CVH Coalition steering committee once established to expand the Ohio CVH Coalition membership. A stroke subcommittee will be formed, and stroke data and quality improvement findings will be shared with coalition members in Years 2-5. (5) Ohio Coverdell will expand the community health worker (CHW) Stroke Coach Pilot conducted under DP21-2102. Under DP21-2102, ODH created stroke patient education modules to be distributed via the Pathways Community HUB Institute appropriate for those who experienced a stroke as well as those at high-risk of stroke including those with uncontrolled or undiagnosed hypertension. (6) Ohio Coverdell will promote AHA Stroke Month resources, Million Hearts Live to the Beat resources, and Ohio-created BE FAST messaging to promote stroke prevention and awareness and integrate these resources into QIPs. To achieve the maximum reach and impact, Ohio Coverdell has identified its target population as Ohio adults aged 18 years and older who are acute stroke patients or those at high risk of stroke due to health conditions, behaviors, and high social vulnerability. Program activities are designed to reduce disparities among priority populations and increase utilization of social and support services within partners, health care, and community settings for those at the highest risk of stroke. ODH will leverage resources and expertise through collaboration with other CDC-funded programs/organizations, health equity subject matter experts at ODH, and state-level initiatives. This will allow Ohio to increase the scope of activities and expand infrastructure while minimizing duplication of efforts. Reach and impact will be maximized through implementation of sustainable strategies for the prevention and management of stroke.
