Paul Coverdell National Acute Stroke Program - North Carolina (NC) has a disproportionate number of residents who are at high risk for stroke events, and stroke is the fourth leading cause of death in the state. We will develop statewide messaging and education on the importance of calling 911 at the recognition of stroke symptoms. The NC team will collaborate with the NC Health Information Exchange (HIE) to track and analyze data from the HIE stroke registry including using the risk factors interface to identify individuals at greatest risk for stroke with a focus on those with high blood pressure. We will contract with clinics and/or healthcare partners that will monitor data, build and strengthen care teams, develop standardized procedures to track and assess referrals, engage the community-based workforce, and implement quality improvement projects. We will share findings and best practices with stroke stakeholders statewide through quarterly Stroke Advisory Council meetings, monthly Stroke Coordinator meetings, and through the NC Stroke System of Care. We will concentrate on identifying and addressing health inequities by providing statewide training, strengthening partnerships, and in collaboration with healthcare partners across the continuum of care.
