MINNESOTA DEPARTMENT OF HEALTH STROKE PROGRAM - In Minnesota, stroke is the fifth leading cause of death and a leading cause of disability. Over 12,000 stroke hospitalizations occur each year, incurring costs of nearly $400 million annually. There are over 100,000 stroke survivors living in Minnesota. Stroke mortality rates in Minnesota have declined overall since the early 1970s, but in recent years, the rates have flattened. As such, the burden of stroke in the state remains high. The Minnesota Department of Health (MDH) has aggressively expanded efforts to address the burden of stroke. The Centers for Disease Control and Prevention (CDC) Paul Coverdell National Acute Stroke Program granted funding for MDH to launch the Minnesota Stroke Registry in 2007. MDH has worked with several partners to build a nationally recognized stroke registry, program, and statewide system of care over the last 14 years. This application will describe our efforts to meet project objectives and achieve desired program outcomes by building on the past 14 years of partnerships, experience, capacity, and systems developed.
