The Maryland Colorectal Cancer Control Program (MCRCCP) will increase colorectal cancer screening rates among Maryland adults ages 50 to 75 years old, with a focus on target populations with low screening rates. The MCRCCP will leverage key partnerships and its experience implementing system-level changes to improve population health and reduce colorectal cancer disparities, incidence, and mortality. In this five-year project period, MCRCCP will: • Facilitate the implementation of evidence-based interventions in health systems and primary care clinics across Maryland, with a focus on federally qualified health centers in the Greater Baltimore metro area and the Eastern Shore. These clinics reach populations known to have lower colorectal cancer screening rates, including the uninsured, the medically underserved, and minority and rural populations. • Provide health systems and clinics with guidance, tools, and resources to assess clinics’ capacity and readiness to implement evidence-based interventions, and identify at least two evidence-based interventions to implement in each clinic. • Collaborate with Maryland local health departments, Maryland Medicaid, the Primary Care Coalition, the American Cancer Society, and community-based organizations to increase colorectal cancer screening rates in health systems and primary care clinics by: 1. Supporting implementation of evidence-based interventions; 2. Promoting community-clinical linkages; 3. Implementing community-based approaches to reduce structural barriers (e.g. transportation) to colorectal cancer screening; 4. Providing resources to help patients obtain follow-up colonoscopies and treatment; and 5. Sharing quality and data system improvement expertise. • Collect, validate, and monitor clinic-level colorectal cancer screening rates and other related data. • Collaborate with other Centers for Disease Control and Prevention-funded programs in the Maryland Department of Health to align and ensure coordination of cancer prevention and control efforts.
