ABSTRACT In order to prevent and minimize suffering and death from colorectal cancer in Louisiana, the Louisiana Colorectal Health Program (LCHP) will leverage partnerships to increase colorectal cancer screening rates in health systems in areas with high incidence and mortality and late stage of diagnosis rates. The project will assist health systems in implementing sustainable, evidence-based interventions that will improve their ability to conduct, monitor, and sustain systematic screening efforts. The level of suffering and death from colorectal cancer (CRC) in Louisiana (LA) is among the worst in the nation; the state has the fourth highest incidence and mortality rates in the US. (Cancer Control Planet, 2012-2016). LCHP will target people with low screening rates and in areas with high incidence, mortality and late stage of CRC diagnosis rates, including Black men, who have the highest incidence and mortality rate of any ethnicity/gender group. LCHP will work with eight clinical partners to reach these populations, including six FQHCs, and a regional public hospital. In order to help these implement evidence-based interventions to increase screening rates, the project will use several supporting strategies: practice facilitation, patient navigation technical assistance, mini-grants, health infomatics, and professional education. Also, LCHP will engage additional partners to support the implementation of EBIs in clinical partners. These partners include the American Cancer Society, WellCare (Medicare Advantage Plan), Healthy Blue (Medicaid Managed Care Organization), Taking Aim at Cancer in Louisiana, Louisiana Colorectal Cancer Roundtable, Louisiana Department of Health, and the Louisiana Public Health Institute. The main project outcome is increasing in CRC cancer screening rates at clinical partners in accordance with their annual site-specific goals. By end of the five year funding cycle, we also expect at least two clinical partners reach our project’s benchmark screening rate of 80%. Based on progress made by LCHP’s current clinical partners, these are achievable outcomes.
