Fort Peck Community College (FPCC) is applying for a Good Health and Wellness in Indian Country (GHWIC) Component 1 grant. The Fort Peck Healthy Ti¿o´s¿paye project will be based out of the Daya Tibi Health and Wellness Centers operated by FPCC. Based upon the philosophy that health, physical fitness, and spirituality is important in American Indian heritage, the mission of the Daya Tibi Health and Wellness Centers at FPCC is to provide an environment conducive to the promotion of the total health and well-being of the members of the Fort Peck Tribes and citizens of the Fort Peck Indian Reservation. The Fort Peck Healthy Ti¿o´s¿paye project builds upon lessons learned from its previous GHWIC project as well as evidence derived from national efforts. The project proposes to conduct community-based strategies to expand access to healthy foods and beverages, increase physical activity, and to increase engagement in diabetes prevention and self-management and structured treatment programs for those who can benefit from them. The Fort Peck Healthy Ti¿o´s¿paye project proposes a holistic approach to healthy living, utilizing local and culturally-responsive strategies for combating obesity and related chronic diseases – specifically diabetes, heart disease, and stroke. The overall goal of this project is to help reduce rates of death and disability from diabetes, heart disease and stroke, reduce the prevalence of obesity, and to address other behavioral risk factors to chronic disease among American Indians on the Fort Peck Reservation. The purpose of the Fort Peck Healthy Ti¿o´s¿paye project is to promote healthy lifestyle choices by expanding community access to healthy foods along with nutrition education and promoting activity-friendly routes through organized community-based fitness activities. Second, the project engages strategic partners to support and expand the reach of CDC-recognized type 2 diabetes prevention programs in the Fort Peck Reservation communities through community education, screening support with referrals, and culturally-relevant prevention approaches to encourage participation in evolving CDC-recognized type 2 diabetes prevention programs. Finally, the project leverages community partnerships to increase the number of patients at risk for heart disease or stroke enrolled in and participating in self-management and treatment programs. The outcomes expected by the end of the project period are: • Increased number of places offering healthy foods and beverages; • Increased number of people using safe and accessible places for physical activity; • Increased number of community members at high risk for diabetes enrolled in CDC-recognized type 2 diabetes prevention programs offered on the Fort Peck Reservation; and • Increased number of patients with high blood pressure or high blood cholesterol engaged in self-management and treatment programs.
