CRIHB is proposing to implement the Advancing California’s Opportunities to Renew Native health Systems (ACORNS) project to improve the prevention and management and reduce the burden of chronic diseases in California Tribal communities by providing leadership, funding, training, and technical assistance (TA) to California Area Tribes and Urban Indian Organizations (UIO). Through approximately 15 subawards annually, CRIHB will assist its 47 member Tribes and 16 Tribal Health Programs (THPs) to develop, plan, implement, and evaluate projects that focus on preventing and managing obesity, type 2 diabetes, commercial tobacco use, heart disease, and stroke using strategies that incorporate culturally-tailored PSE changes and community clinical linkages (CCLs). Additionally, CRIHB will leverage its 50 years of experience supporting California THPs and Tribes to outreach to all Area Tribes and UIOs to identify statewide training needs on chronic disease prevention and management using culturally-tailored PSE strategies and CCL, coordinate web-based and in-person trainings, and offer tailored TA. CRIHB will also engage Area Tribes and UIOs through meetings, trainings, resource dissemination, and surveys to develop culturally tailored health communication and messaging strategies and multi-sector partnerships.
