Project Abstract The Qawalangin Tribe, a federally recognized sovereign nation in the community of Unalaska, Alaska, has the following mission: to exercise its powers to further the economic and social well-being of all its members, and in doing so, will safeguard and support the Unangan language, culture, and traditions for generations to come. The Qawalangin Tribe is faced with a disproportionate number of health disparities, including, but not limited to: type 2 diabetes, obesity, heart disease and stroke, and high tobacco use. Recent (2018) statistics reveal that among the Alaska Native adult population in Unalaska: nearly one quarter was identified as either prediabetic or diabetic; over one quarter had high blood pressure, and over half of the adult population was reported as obese. These health disparities are likely linked to the remoteness of the community, historical trauma, and lack of access to full medical services. Many services available in larger urban areas are not available to people in Unalaska, and this plays a toll on human health. This project seeks to address the inequities in human health in Unalaska and address them through strategies aimed at: obesity prevention, diabetes prevention, and heart disease and stroke prevention. The long-term intended outcomes of this project will be: increased physical activity with an emphasis on walking; reduced incidence of type 2 diabetes; and reduced prevalence of high blood pressure and high blood cholesterol. The outcome of increased physical activity will be addressed through connecting activity-friendly routes with everyday destinations in Dutch Harbor and Unalaska. Culturally informed adaptations of the routes and walkways will be an integral and an important aspect of the planning and implementation process. The outcome of reducing type 2 diabetes will be addressed through building the local capacity of the community, in partnership with the clinic and City of Unalaska, to implement diabetes prevention classes. The tribe will work closely with partners to develop a culturally adapted diabetes prevention program that attracts community members to participate and stay involved. Finally, the tribe will address the outcome of reducing the prevalence of high blood pressure and high blood cholesterol, by working closely with the clinic and expert partners, to develop a community-clinic linked program for self-monitoring of blood pressure and blood cholesterol. Culturally-based strategies and incentives will be utilized throughout to encourage community engagement and retention. The addition of these local community-based programs and initiatives will greatly help reduce the health inequities and resulting disparities faced by the Qawalangin Tribe in Unalaska, consequently fulfilling the mission of the tribe.
