Sewa U’usim Good Health and Wellness Abstract
The Pascua Yaqui Tribe (PYT), a federally recognized Tribal Government, submits this request for funding through its Sewa U’usim Community Partnership (SUCP), a program of the PYT Health Services Division. We are seeking consideration for five-year funding under the Good Health and Wellness in Indian Country Program (GHWIC) (Centers for Disease Control and Prevention), CFDA #93.479, to expand, develop, and refine the work begun with the assistance of the current small pass-through GHWIC funding received via InterTribal Council of Arizona (ITCA). The proposed Sewa U’usim Good Health and Wellness Project (SUGHW) will reach 4500 community member and will facilitate the continued development, expansion, and refinement of a comprehensive, collaborative multidisciplinary network of support services to increase breastfeeding, reduce the incidence of type 2 diabetes, and reduce the prevalence of high blood pressure and high blood cholesterol among tribal members. This network will be built upon existing services provided by the PYT Health Services Division, WIC, Community Nursing, Maternal Health, Sewa U’usim Community Partnership, and Education Language and Culture Departments to create a seamless evidence-informed and culturally-adapted array of support services on the New Pascua Reservation located southwest of Tucson, AZ.
The proposed SUGHW will implement evidence-informed and culturally-adapted policy, system, and environmental changes to prevent obesity by increasing continuity of care/community of support for breastfeeding; implement evidence-informed and culturally-adapted community-clinical linkages to support type 2 diabetes prevention by developing culturally-relevant approaches to increase program participation and retention; and implement evidence-informed and culturally-adapted community clinical linkages to support heart disease and stroke prevention by developing culturally-relevant materials and approaches to link tribal resources and clinical services to support prevention, detection, and control of high blood pressure and/or high blood cholesterol. The purpose of the SUGHW is to enhance tribal community awareness and participation in programs that promote health and wellbeing and build resiliency in children youth and families. It aims to increase PYT members’ sense of connection to community, family, and culture utilizing the PYT Heart Loss/Thought Loss Approach and Good Order Chart, will lead to healthier Yaqui families and communities. This will lead to healthier Yaqui families and communities and result in the reduced rates of death and disability from diabetes, heart disease, and stroke, and decreased prevalence of obesity and other chronic disease risk factors and conditions among the PYT members.
The performance measures to be monitored for the period of performance include: Number of service programs that establish lactation support services as a result of project; Number of participants at the Community Baby Shower; Number of participants who show improvement in pre-/post-test knowledge assessment; Number of Mother/Families that participant in the peer to peer support groups; Number of Breastfeeding Policies developed; Number of Departments developing/ implementing policies; Number of participants engaged in program services; Number of prevention programs offering culturally relevant materials/approaches as a result of this project; Number of participants engaged in program services; Amount of knowledge increase from pre-/post-test knowledge assessment; Number of Health Literacy materials/approaches developed and distributed; and Number of PSAs developed and aired on Pascua Yaqui Tribal Radio.
