Abstract This application outlines how the Republic of Palau Ministry of Health will work with internal and external partners to prevent and control tobacco use, diabetes, and cardiovascular in the communities. This application is comprised of the two components: the Core Component and the optional Oral Health Component. Palau Ministry of Health (applicant) will support the implementation and evaluation of a core set of evidence-based strategies designed to address tobacco use and secondhand smoke exposure, diabetes management, type 2 diabetes prevention, and cardiovascular disease prevention and management. Proposed in this application are plans that address each of these areas in alignment with the four strategies indicated in the NOFO. Palau Ministry of Health also opts to include the oral component in this application to support the implementation and evaluation of a focused set of activities designed to improve oral health capacity and better address the oral health needs of Palau’s communities.
