Chronic diseases – such as diabetes, heart disease, and stroke – are the leading causes of death and disability in the United States. And, more than four out of every five health care dollars spent in the U.S. are spent on people with one or more chronic conditions.1 Wisconsin’s diabetes and heart disease burden are impactful. In Wisconsin, 7 out of 10 leading causes of death are due to chronic diseases, accounting for two out of every three deaths annually.2 Nearly 10% of Wisconsin adults have diagnosed diabetes, an increase of 17% since 2011. In addition, 12% of Wisconsin adults have diagnosed prediabetes and are at an increased risk for developing type 2 diabetes, heart disease, and experiencing a stroke. Approximately 1.3 million adults in Wisconsin have hypertension, but less than half of them are in control. Of those who are uncontrolled, 40% are unaware that they have hypertension.3 The work of Wisconsin’s Chronic Disease Prevention Program (CDPP) and our partners under Innovative State and Local Public Health Strategies to Prevent and Manage Diabetes and Heart Disease and Stroke (CDC-RFA-DP18-1817) will reduce the burden of diabetes and cardiovascular disease (CVD) through the design, testing, and evaluation of novel approaches to address evidence-based strategies aimed at reducing risks, complications, and barriers to prevention and control of these diseases in high burden populations and communities. Eau Claire County, the geographical focus of the proposed diabetes prevention work in this grant application, has Wisconsin’s third highest age-adjusted diabetes prevalence rate.4 Committed partners that include the region’s largest health systems, local public health departments, community-based organizations, insurers, and large employers will work together to increase enrollment in the National Diabetes Prevention Program through development of bi-directional e-referral systems, creation and testing of tailored promotional messages, development of a Peer-to-Peer Learning Network for Lifestyle Coaches, exploration and development of solutions to eliminate participation barriers, and the testing of telehealth models to best serve this rural population. We will implement the Pathways Community HUB (HUB) as a comprehensive, data-driven approach for CVD prevention and management. The HUB will improve care coordination through the use of community health workers (CHWs) and provide strong infrastructure for evaluation and assessment. The innovative nature of the HUB targets high-burden populations by addressing social determinants of health, encourages value-based payment structures, and increases community-clinical linkages and bi-directional referrals for CVD management and prevention. Committed partners include existing HUBs, FQHCs, free clinics, several health systems, and several health departments. UW- Population Health Institute will serve as lead evaluator for the grant. This work will successfully lead Wisconsin to an increased number of people with prediabetes enrolled in a CDC-recognized lifestyle change program who have achieved 5-7% weight loss; increased control among adults with known high blood pressure; and, increased cholesterol management among patients at high risk for cardiovascular events over the next five years.
