Soil transmitted helminths (STH), transmitted through sewage-contaminated soil globally, continue to inflict tremendous disability and suffering, yet can be controlled or eliminated. STH infections lead to chronic poor health through anemia and nutritional deficiencies compounded by poor nutrition and other infections. Historically, STH infections were highly prevalent in the southeastern US, and were then considered eradicated. Recently however, there has been recognition that conditions to support STH infection persist, and recent small studies support this. To address STH and widespread inadequate sanitation in Alabama’s Black Belt region, we propose to expand the work begun in rural Alabama as part of our previous CDC-funded surveillance to identify areas at risk for STH infections, establish whether STH remain endemic, and to implement control strategies. We propose expanding surveillance and treatment activities from 3 to 8 counties and include all age groups across this region deemed at risk due to a high prevalence of failing household sanitation. We plan to establish a comprehensive local infrastructure through partnerships/collaboration with local stakeholders, healthcare providers (HCPs), and healthcare-focused community agencies to improve knowledge and awareness among both HCPs and the community about diagnosis, treatment, and risk factors for STH through educational programming and connection to available resources. Our short-term outcomes include: Improved understanding of STH epidemiology in rural Alabama Improved HCP knowledge for STH prevention, risk factors, diagnosis and treatment. Availability of tools / training materials for HCPs, patients and community on STH and sanitation risks and prevention. Enhanced local capacity for ADPH and health clinics to identify, control and prevent STH. Our intermediate outcomes include: Improved HCP and community awareness of STH prevalence, risk and control programs. Availability and dissemination of effective surveillance methodologies and prevention programs. Our long-term outcomes include: Eradication of STH in rural Alabama. Overall improvement in wastewater management in rural Alabama with the resultant decrease in disease morbidity associated with inadequate sanitation. To accomplish this we will: 1) Expand STH surveillance and treatment by using existing healthcare infrastructure and clinical diagnostic services to enhance capacity through collaboration, education, and support personnel; 2) Develop educational programming and survey tools to assess baseline HCP knowledge of STH and the community. Then educational materials will be created and disseminated; 3) Disseminate the results from our work including epidemiologic investigation, surveillance methodologies, impact assessment and trends through publications and presentations at relevant meetings with project partners. 4) Establish a robust, sustained surveillance and eradication program and solutions to inadequate wastewater management to prevent further STH disease throughout rural Alabama. We will monitor the effects of our control program focused on identification of areas of disease, effective treatment, and implementation of improved wastewater management throughout the region. Social workers in the target communities will disseminate resources from this project to assist households apply for state and federal programs to improve or replace inadequate household sanitation systems.
