Comprehensive ESSHCI Project for Metro Columbus, Ohio - Project Title: Comprehensive ESSHCI Project for Metro Columbus, Ohio Lutheran Social Services of Central Ohio is the legal applicant. Its clinic in downtown Columbus - LSS Health Center - will implement the project. LSS Health Center’s overall approach and activities are consistent with CDC’s purpose and logic model. Its strategies are evidence-based, its activities are achievable and appropriate to achieve project outcomes, and it has the distinct ability to reach the priority populations disproportionately impacted by STIs. LSS Health Center currently offers all of the Basic STD Services, and its proposed staffing plan and organizational capacity are sufficient to increase identification of new STIs, improve the patient clinic experience, increase linkage to public health partners, and support CDC’s logic model and data reporting requirements to measure progress toward the project’s long-term outcomes. LSS Health Center will implement Strategies A, B, and C. Purpose: The purpose of this application is to achieve optimal sexual health for all adults in a high burden area for STIs, particularly for LSS Health Center’s priority populations who are disproportionately impacted by STIs. LSS Health Center is committed to involving representatives from its priority populations to address sexual health disparities and to supporting the goals and objectives of its public health agency partners and CDC’s National Center for HIV, Viral Hepatitis, STDs and TB Prevention office. Outcomes: The period of performance outcomes for 9/30/2023 to 9/29/2024 include the following: 1. Increased community involvement in clinic-level planning for provision of comprehensive sexual health services 2. Increased engagement with public health partners addressing the STI epidemic 3. Increased clinic capacity to provide quality comprehensive sexual health services 4. Increased access to quality, stigma-free STI care and comprehensive sexual health services, especially among priority populations affected by STIs 5. Improved patient clinic experience 6. Increased identification of new STIs by clinics 7. Increased linkage to public health partners for prevention and care services for cooccurring conditions 8. Increased access to quality, stigma-free STI care and comprehensive sexual health services, especially among LSS Health Center’s priority populations affected by STIs. The priority populations for this project include homeless adults and victims of intimate partner violence who lack stable housing, adults with Substance Use Disorder and Co-Occurring Disorder (SUD/COD), and minority adults living with or at high risk for HIV/AIDS infections.
