Strengthening STD Prevention and Control for Health Departments (STD PCHD) - California contributes disproportionately to the nation’s STD burden and is experiencing record increases in all reportable STDs. In 2017, 218,710 chlamydia (CT) cases, 75,450 gonorrhea (GC) cases, and 13,605 primary and secondary syphilis (P&S) cases were reported in the California Project Area (CPA), representing respective increases of 88%, 280%, and 767% compared to our case numbers in 2012. The rate of early syphilis (ES) among females rose to 18.4 per 100,000 in 2017, a nearly 7-fold increase since 2012. There were 278 cases of congenital syphilis (CS) reported in 2017, including 30 stillbirths, representing the fifth consecutive year of increases and our worst magnitude of CS burden since 1995. Our application describes robust and strategic surveillance, disease investigation (DI), clinical care, prevention, policy, and data utilization activities that we will implement to reduce the incidence and impact of STDs, including HIV, in the CPA. While direct client services are delivered at the local level, the state provides guidance, coordination, local capacity building (LCB), surge and safety-net services to local health jurisdiction (LHJ) STD/HIV partners and other key stakeholders, facilitates innovation, and builds strategic collaborations to align efforts across sectors. Our proposed activities will strengthen STD surveillance by improving the quality of case data reported electronically from laboratories. We will modernize and streamline our outbreak identification, communication, and response systems to ensure our efforts are robust, coordinated, and timely. To improve the efficiency and outcomes of DI efforts, we will focus on LHJ LCB and surge support, and will assess existing local DI staffing and resources to inform prioritization recommendations. We will fund prioritized LHJs based on CS morbidity to enhance CS prevention activities through case review via local morbidity and mortality review boards, case management and linkage to care, public health detailing, and by improving syphilis screening among incarcerated women of child-bearing age. To expand the provision of expedited partner therapy (EPT) to CT and GC cases, we will educate providers, promote our free EPT medication program, and identify and address policy barriers related to onsite dispensing. To promote quality STD specialty care services, we will enumerate categorical STD clinics and identify and address key gaps in clinical best practices. We will assure provider adherence to GC and syphilis treatment recommendations, and will develop a mechanism to forecast Bicillin L-A® shortages and implement an emergency response plan for safety-net distribution of the medication to LHJs. Our efforts with primary care practices, health plans, and other key healthcare sector stakeholders will aim to improve sexual health care and STD screening for adolescents and young adults, and we will support STD clinical trainings in high-priority LHJs. We will improve STD services for gay men and other men who have sex with men in HIV care settings and will engage the growing number of providers offering HIV pre-exposure prophylaxis (PrEP) in order to improve concurrent STD screening. We will support the implementation of California’s new comprehensive sexual health education law by expanding our training program for public school teachers in partnership with the California Department of Education and the California Prevention Training Center. We will monitor new proposed policies that impact STD prevention and control, and will promote policies that aim to improve access to healthcare for our priority populations, ensure comprehensive sexual health education, and reduce stigma. Finally, we will continue to build out our data utilization efforts to better drive and target our program strategies for the greatest impact on reducing STD transmission and adverse outcomes in our project area.
