Seedco, a proud recipient of CMS 2021 Cooperative Agreement to Support Navigators in Federally-facilitated Exchanges proposes to provide outreach and education about Exchange-based coverage, help individuals to establish eligibility and enroll in coverage, and determine if they may qualify for affordability programs in a 10-county region in western Tennessee for the 2023-2024, Non-Competing Continuation Application. The proposed Year Three program targets underserved and vulnerable populations, including racial and ethnic minorities, low-income individuals and families, those with HIV, rural populations and others. Seedco seeks to leverage its 12 years of experience in implementing outreach, education and enrollment assistance services for Affordable Care, Medicaid and Children's Health Insurance Program (CHIP). Seedco will be the sole entity in its proposed Navigator program.
Target Counties in the State of Tennessee: Chester, Dyer, Fayette, Hardeman, Haywood, Lauderdale, McNairy, Madison, Shelby and Tipton Counties.

Project Goals
Seedco will increase capacity for Navigation services by maintaining the 6 FTE Navigators. This will support the increase in the expansion of awareness of available Navigator services, a critical need during this time of the PHE Unwinding. Seedco will also increase enrollments in a Qualified Health Plan (QHP), Medicaid or Children's Health Insurance Plan (CHIP) while increasing awareness of the requirements to provide post enrollment information on the appeals process, Premium Tax Credit information and more.

Target Populations for 2023-2024 will include the uninsured (138,506); Minorities: Black (597,500) and Hispanic (70,271); Rural population: 210,950; Low income: 209,987; Pregnant women, new mothers and women with children: 135,889, women between 20-34 years of age; individuals with HIV/AIDS: 7,302; Medicaid eligible that are not enrolled: 60,139.
Proposed Project Plans: Seedco has twelve years' experience in implementing state and federal Navigator programs, understanding and meeting standards for services as established in federal statutes. The program will meet all requirements to serve the needs of the consumers first, including providing fair, accurate and impartial information: providing services to anyone seeking assistance; implementing conflict of interest standards; providing information to consumers about the full range of options for which they are eligible.
