(MRMC), the grantee of record and the parent company for Lewis Health Center (co-applicant FQHC) has been in operation since 1953 as a non-profit health system and currently operates 7 clinic locations, serving the low-income and un/underinsured people of Lewis, Marshall, Maury, and Wayne counties of Tennessee. MRMC’s service area faces significant challenges to health; namely, high rates of poverty and high rates of uninsured. Median household income among Lewis Health Center’s service area is relatively low compared to state averages and the poverty rate (17.5%) is higher compared to the state rate (15%), as well as the social determinants of health that adversely impact the community which is evidenced by high rates of chronic diseases, mental illness and substance use. Lewis Health Center offers primary care for adult and children, imaging, laboratory and physical therapy services to all patients within the community. Primary care services include illness, management of chronic disease, injury treatment and preventive care, including vaccinations and wellness exams, and are offered via appointment or walk-in and are offered both in-person and virtually, meeting each patient where they are. For the convenience of patients, Lewis Health Center offers on-site point of care services. Additionally, newborn exams, infant and child sick visits, immunizations, wellness exams and sports physicals are offered to pediatric patients. Supportive services are also offered to patients to ensure full access to care, including but not limited to, transportation, translation services and specialty referral services including dental care. Lewis Health Center is proud to serve the rural Tennessee community and will continue to develop and expand on its programming to ensure equitable access to care that meets the needs of the service area population. In 2023 alone, LHC served 20,681 unique patients through its FQHC Clinics. LHC provided 2,630 clinic visits and 644 virtual visits for those with a mental health diagnosis, serving 745 unique patients. LHC did not provide SUD services directly in 2023, a strategic initiative for the FQHC. The Health Center will focus on integrating its robust behavioral health programming with SUD programming through BHSE funding. LHC will leverage HRSA BHSE funds to increase access to behavioral health services through the expansion of mental health and substance use disorder (SUD) services. Project activities include: - Leveraging community partnerships to better serve underserved populations (those who are uninsured, persons experiencing homelessness); - Integrating primary care, dental, and behavioral health, conducting assessments and connecting clients with Peer Support Specialists to expand access to supportive services; - Addressing social drivers of health through community collaborations (housing insecurity, food insecurity, financial strain); - Expanding the role of primary care providers in the use of FDA-approved medications for treatment of opioid use disorder through training and education; and - Hiring additional behavioral health staff to expand access, meet the growing demand. Program goals to be achieved via the project activities listed above include: - Increase the number of patients receiving mental health services; and - Increase the number of patients receiving SUD services, including treatment with medications for opioid use disorder.
