Comanche County Hospital Authority dba Lawton Community Health Center (LCHC) will specifically address breast, cervical and colorectal cancer screening and referral for treatment through the AxCS project therefore increasing access to cancer care, resulting in improved health outcomes and overall community health. The organization will provide cancer screening services to address the community's health care disparities by focusing on cancer which is disproportionately represented in the target population. The organization will utilize $500,000 in federal funds to expand cancer screenings and streamline positively screened patient’s referral to diagnostics or treatment. LCHC will utilize two individuals (1.0 FTE licensed nurse and 0.5 case manager BSW) in the LCHC population health department to review cancer screening data reports and contact patients via multiple methods (phone, letters and text messages) on the need for screenings as well as work with the providers and cancer center navigators to get positively screened patients in for treatment or additional diagnostic testing. Additionally, LCHC plans to add 3.0 FTE medical technicians who will assist in screening patients for in office testing or in setting up referrals and any required authorizations for test done at another facility. LCHC will add a part-time data analyst who will assist in running reports from the electronic health record to identify those individuals needing testing in which testing has not been performed. LCHC cervical cancer screenings, mammogram to screen for breast cancer and colorectal cancer screenings increased during FY 2020 and FY 2021. By the end of the AxCS project period, LCHC anticipates the following screenings to increase: 50% of women 23-64 years of age will receive cervical cancer screening, 80% of women age 51-73 years old will receive mammogram screenings and 70% of patients age 50-74 years old will receive colorectal screenings. LCHC will
improve and expand the organization's cancer screening services for residents who live in Comanche County, Jefferson County, Stephens County and 8 zip codes within Kiowa County and Washita County. Oklahoma. The organization provides an integrated and comprehensive health care team, care coordination, and sliding fee discount scale according to one's income level.
