Health Center Program - Purdue University North Central Nursing Clinics (NCNC) is a Federally Qualified Health Center (FQHC) operated in partnership with Purdue Universitys School of Nursing. Established in 1995, the Family Health Clinic of Carroll County was created to address the healthcare needs of individuals facing financial and access barriers. In 2009, a strategic partnership with North Central Nursing Clinics, an independent non-profit organization, secured federal Health Resources and Services Administration (HRSA) funding, enabling the clinics to attain FQHC status. Today, NCNC operates across four sites in the medically underserved communities of Burlington, Delphi, Monon, and Wolcott, with its operational headquarters located in Delphi, Indiana. Collectively, these sites serve residents of Carroll and White counties, focusing on populations with limited access to healthcare. Guided by a mission to deliver high-quality, accessible, and affordable primary care, the clinics also serve as vital training sites for future health professionals. They offer comprehensive services, including routine medical care, womens, mens, and childrens health, immunizations, pharmacy services, and behavioral health support. Dedicated to reducing health disparities, NCNC actively serves uninsured patients through a sliding fee scale, ensuring that cost is not a barrier to essential care. The character of the service area is overwhelmingly rural, and it is located in north central Indiana. The four sites serve 45 ZCTAs (Carroll and White counties). The service area population includes 45,580 residents, including 13,431 low-income individuals. In 2024, NCNC served 9,026 unique patients, an increase of 308 additional patients from 2023, providing 25,699 in-person clinic visits and 809 virtual visits, including medical, mental health, and enabling services. Of this number, 90.79% of NCNCs patients were at, or below, 200% of the federal poverty level (FPL), and 10.99% of all patients were uninsured, 39.32% of all patients were on Medicaid, 14.60% were on Medicare, and 15.49% of patients were best served in a language other than English. Furthermore, Medically Underserved Areas and Health Professional Shortage Areas are present throughout the service area, indicating need. NCNC has proudly served the community for over 30 years, focusing on high-quality care, prevention, continuous compliance, and the overall well-being of the local population. In 2014, NCNC earned the National Committee for Quality Assurance (NCQA) Level 2 Patient-Centered Medical Home designation, reflecting a commitment to care coordination. NCNC received the HRSA Community Health Quality Recognition (CHQR) award for advancing health information technology and, in 2025, achieved another CHQR badge as a National Quality Leader in heart health. Additionally, NCNC conducts mobile outreach efforts that provide free food, personal hygiene, and household products to individuals in need within the service area, further extending its reach and support to underserved communities. These honors showcase NCNCs dedication to excellence, innovation, and community well-being. Continued FQHC designation will allow NCNC to continue to serve the target population, providing care to at least 9,956 patients by the end of the designation period. NCNC will continue to deliver existing services such as comprehensive primary and preventative medical care, pharmacy, mental health, case management, and enabling services responsive to the target populations needs.
