Health Center Program - Migrant Health Center, Western Region Inc. (MHC) is a distinguished private, non-profit 501(c)(3) tax-exempt, Federally Qualified Health Center (FQHC) that has been providing essential primary and preventive healthcare services since 1971. Situated in Puerto Rico’s (PR) Western Region (WR), MHC operates fifteen (15) primary health care facilities across ten (10) municipalities. As the sole “one-stop-shop” healthcare provider in the area, MHC offers a wide range of primary and preventive healthcare services to underserved populations, regardless of their financial status or ability to pay. The comprehensive services provided by MHC meet all essential healthcare requirements set by the Health Resources and Services Administration (HRSA). These services include general primary care, laboratory tests (both in-house and home visits), radiology, screenings, family planning, immunizations, gynecological and obstetrical care, dental services (in-house and mobile clinics), pharmacy facilities (in-house and home delivery), case management, mental health support, substance abuse treatment, nutritional guidance, optometry, health education, community outreach, eligibility assistance, transportation aid, and home visits. MHC’s noteworthy achievements include the accreditation of twelve of its fifteen clinics with the National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) recognition, HRSA Health Center Quality Leaders 2022 (Gold), various gold recognitions from the American Heart Association in 2023, recognitions of excellence from the local municipalities, where our centers provide services, and PR Best Employers Recognition (2023). The center participates in the 340B Drug Pricing Program and is designated as an FTCA deemed organization. Renowned as a center of excellence by the PR Health Care Association and the PR Department of Health, among others, MHC collaborates with both public and private entities to offer additional services and maintains formal agreements for contracted services and continuity of care. The center also serves as a teaching institute for the University of PR, The Ponce School of Medicine, and the “Universidad Catolica de PR.” According to the US Census data (2021) update, as of September 2021, PR's poverty rate remained significantly higher compared to the US national average. Specifically, PR's poverty rate was approximately 42.2%, while the US had a rate of about 12.5%. Serving an area in PR’s western region, where approximately 80% of the 309,395 residents live below the Federal Poverty Level (FPL), MHC plays a crucial role in addressing the healthcare needs of the community. The center’s most recent needs assessment reflects that cardiovascular diseases, cancer, diabetes, hypertension, asthma, obesity, HIV/AIDS, and mental health issues continue to be the primary health concerns in the region. According to MHC’s UDS 2023, the center provided comprehensive primary and preventive healthcare services to 36,019 unique patients. These patients accounted for a total of 146,739 medical encounters, with a substantial majority (97%) living at or below 200% of the Federal Poverty Level (FPL). The patient population at MHC has experienced a notable growth of (21.2%) from 2021 to 2023. This increase highlights the organization's unwavering commitment to expanding access to healthcare, enhancing the quality of services, and engaging in robust outreach and enrollment initiatives. These efforts are part of MHC's ongoing mission to address and adapt to the evolving healthcare needs of the community it serves. Moving forward, MHC estimates its patient trend to continue, with no less than 5% increase in patient population in the next three (3) years. To continue its vital work and fulfill its mission, MHC is seeking $9,579,593 in federal funding to support and enhance its healthcare services for the underserved populations in Puerto Rico’s Western Region.
