Established in 1978, SEMOHN has expanded to include 8 medical clinics, 4 dental clinics, and a dental mobile unit. These facilities serve a six-county area in Missouri’s rural bootheel, encompassing Dunklin, New Madrid, Mississippi, Pemiscot, Scott, and Stoddard Counties. SEMOHN delivers essential primary medical, dental, and mental health services to low-income, uninsured, and underinsured individuals and their families across roughly 3,364 square miles. The total population of our service area is 140,104 of which approximately 45.39% live at or below 200% of the Federal Poverty Level, creating a target population of 63,597 individuals. In addition to the above services, SEMOHN also focuses on providing care to migratory and seasonal agricultural workers (MSAW) and their families, ensuring that this vital population has access to necessary health services. Currently, SEMOHN employs a dedicated team of 155 full and part-time professionals, including ten family nurse practitioners, two physician assistants, two MDs, two DOs, three behavioral health providers, and six dentists. SEMOHN offers a comprehensive array of services, from primary health care and oral health care to diagnostic laboratory and radiology services, case management, eligibility and marketplace assistance, 340B drug program management, outreach programs, migrant services, and translations services, among others. Last year, SEMOHN provided care to 17,242 unique patients resulting in over 50,000 visits. A significant portion of SEMOHN’s service area residents experience ongoing poverty, with many residing in some of Missouri’s most economically challenged counties. A large number of our patients rely on Medicaid or participate in our sliding fee program. Health disparities among our target population surpass national and state benchmarks in chronic illness and at-risk behaviors, highlighting the critical need for primary health and dental care services focused on chronic disease management and innovative, patient-centered care. These counties consistently rank poorly in major health metrics such as health outcomes, life expectancy, quality of life, and health factors. SEMOHN is in a unique position to address these challenges and enhance the quality of life across the counties and communities we serve. To further our mission, SEMOHN seeks grant funding from HRSA’s Service Area Competition funding opportunity totaling $3,874,731.00 to expand services and reach a minimum of 17,242 unique patients by December 31, 2026.
