Northern Cheyenne TOR Project - The Northern Cheyenne TOR Project aims to address opioid and other substance abuse and promote wellness within our community. Our proposed community defined evidence practices are founded on culturally competent practices as we seek to integrate traditional knowledge and modern interventions to enhance recovery, prevention, and harm reduction efforts. The following represents our Primary Goal and Measurable Objectives for our TOR project: Primary Goal: To provide effective recovery, prevention, and harm prevention/reduction services combined with culturally relevant activities to reduce the prevalence of OUD on the Northern Cheyenne Indian Reservation. Objective #1: By the end of month three of the project period, the NCTH will advertise and hire a qualified professional services firm with experience in substance use disorder programs and working with Native American communities to update our existing Opioid Strategic Plan as well as provide external evaluation services. This objective is specific through the hiring of a professional services firm, measurable through the contract for services, achievable through experience of the contractor, relevant to the project's goal and objectives, and time-bound by the contracts scope of work. Objective #2: Beginning in month four and continuing throughout the five-year project period, the NCTH will provide culturally relevant recovery support services to 100 unduplicated individuals (5-year total) recovering from OUD and co-occurring SUDs. This objective is specific in targeting individuals with specific needs, measurable through tracking unduplicated service recipients, achievable by leveraging existing resources and partnerships, relevant to the community's pressing need for comprehensive recovery support strategies, and time-bound to achieve the target within the project timeline. Objective #3: Beginning in month four and continuing throughout the five-year project period, the NCTH will implement culturally informed and responsive evidence-based prevention efforts to at least 100 unduplicated individuals (5-year total), integrating teachings from Northern Cheyenne Elders. This objective is specific in its approach to prevention, measurable through annual outreach metrics, achievable through tailored programs, relevant given the high prevalence of prescription drug misuse, and time-bound to ensure ongoing prevention efforts over the project's duration. Objective #4: Beginning in month four and continuing throughout the five-year project period, the NCTH will provide culturally guided harm reduction services to 100 unduplicated individuals (5-year total), including quarterly community events in all five (5) Tribal communities that will serve as our primary outlet for community engagement and TOR related trainings. This objective is specific in targeting harm reduction strategies, measurable through service provision data and stakeholder training outcomes, achievable through program development, relevant to the urgent need for harm reduction strategies, and time-bound to deliver services consistently over the project's timeline. Objective #5: Throughout the entire five-year project period, the NCTH will achieve a GPRA/SPARS target of 300 unduplicated individuals (5-year total) with a plethora of recovery support, prevention, and harm reduction services guided by culturally informed practices by the end of Year 5 (60 unduplicated per year). This objective is specific in providing a diverse range of culturally driven evidenced based services, measurable through tracking service utilization, achievable through a coordinated approach, relevant to addressing the opioid crisis comprehensively, and time-bound to achieve the target within the project's timeframe.
