As COVID-19 has created a new normal, MHMR of Tarrant County (MHMR) proposes the Tarrant County Community Mental Health Center (CMHC) Program to ensure the safety of staff and clients with SMI, SED, and COD who have also been impacted by the COVID-19 pandemic in Tarrant County. As the third-largest county in Texas with over 2.08 million citizens and the 15th largest county in the United States, there is high demand to proactively serve the most vulnerable populations in the community. For example, within the last year, the Fort Worth Police Department (FWPD)'s internal data has seen a spike in mental health and suicidal ideation crisis calls, which include frequent callers who may only need a wellness check instead of police dispatch. Additionally, the pandemic's disruption of face-to-face service delivery, in-person and office follow-ups, and encouragement of physical distancing has increased stress and created additional strains among the most vulnerable populations. Therefore, the Tarrant County CMHC Program will broaden capacity for the provisions of integrated behavioral health and primary care access to adults and youth individuals with SED, SMI, and COD that have been severely impacted by the COVID-19 pandemic. Additionally, the program will proactively deliver immediate follow-up visits to people with mental health needs that call 911 but do not require law enforcement intervention. A collaborative effort between MHMR's Divisions (Behavioral Health Services, Child and Family Services, and Disability Services) and the FWPD, referrals will be received through agency points of entry, including Outreach and Navigation, its Law Liaison Program (LLP), and the ICARE Call Center (ICARE). ). This project will include a Project Director, Program Manager, Prescriber-Advanced Practice Registered Nurse, Law Liaison, START Crisis Intervention Specialist, START Coordinator, Help Me Grow Navigator, Mobile Crisis Outreach Crisis Interventionist, Call Center Crisis Interventionist, Rehab Case Manager, Family Interventionist, DS Service Coordinator, and a Mental Health Peer Support Specialist. This team will provide services to address SMI, SED, and COD, as well as COVID-19 resource information to those in need of critical support. Staff will ensure follow up is completed, facilitate access to care, and ensure continuity and linkage to community resources. All people linked to MHMR integrated services will receive care using over 10 evidence-based practices, with their unique needs assessed as needed to determine the most appropriate form of care. As a certified CCBHC and the largest provider of behavioral health care in Tarrant County, MHMR has an established ability to effectively provide care for SMI, SED, COD, and other primary health care needs. Through the Tarrant County CMHC Program, MHMR plans to serve 1,000 unduplicated individuals during the two-year project period. By gathering and exchanging data, MHMR and FWPD staff on will utilize the information available in the servers to carry out effectively, respond adequately, and assure client, officer, and community safety.
