Ryan White Part C Outpatient EIS Program - Address: 1500 21st Street, Sacramento, CA 95811-5216 Project Director: Michelle Monroe, CEO Phone: (916) 443-3299 Fax: (916) 325-1984 Email Address: mmonroe@onecommunityhealth.com Website:www.onecommunityhealth.com Funds Requested for Ryan White Part C-EIS: $580,742 Brief Description of the Proposed Project: Cares Community Health dba One Community Health (OCH) is a federally qualified health care center that has received Part C-EIS funds since 2001 and serves the proposed service area of Sacramento, El Dorado, Yolo, Alpine, Placer, Sierra and Nevada Counties. In addition, OCH’s service area includes surrounding inland California counties as far north to the Oregon border and east to the Nevada border. Most of this area is defined as Rural by the U.S. Census Bureau. OCH’s service area includes the Ryan White Part A Transitional Grant Area (TGA) consisting of El Dorado, Placer, and Sacramento Counties. Part C funds are used at OCH as a payer of last resort and are an essential base to support our RW-Part C Early Intervention Services and Core Medical and Support Services. Needs to Be Addressed: It is critical that OCH to be able sustain continuity of care for these underserved minority patients who are heavily impacted by HIV/AIDS, and who rely on RW Part C funding to maintain the necessary continuity of care. OCH serves several rural counties outside of the RW Part A TGA, specifically Yolo, Alpine, Sierra, and Nevada counties. Current RW Part C funding provides EIS services to these rural areas. OCH’s burden of caring for PLWH has increased due to patients with intense needs resulting from their socioeconomic situation, including poverty, homelessness, substance use, previous incarcerations, mental illness, an aging patient population, and patients with co-occurring medical disorders which leaves them without adequate resources to care for their HIV disease. Also, the expense of maintaining the standard of care and improving health outcomes for uninsured and underinsured patients continues to grow. Proposed Services: The Ryan White HIV/AIDS Program’s (RWHAP) Part C Project proposed aims to ensure that the target population receives the medical and support services that are not available to these individuals elsewhere in the greater Sacramento region. These services include Outpatient Ambulatory Care Services, EIS, Health Insurance Premium & Cost Sharing, Medical Case Management, Mental Health Services, Medical Nutrition Therapy, Oral Health Care, Substance Abuse Outpatient Care Services, Emergency Financial Assistance, Food Bank/ Home-delivered Meals, Health Education/ Risk Reduction, Medical Transportation, Non-Medical Case Management, Housing, and Outreach Services. Population Groups to Be Served: According to One Community Health’s 2023 Ryan White Services Report, approximately 1,163 (55.3%) OCH clients served live at or below 100% of the Federal Poverty Guidelines with nowhere to turn for medical services for HIV/AIDS and 33% of clients are people of color. It is OCH’s goal to continue to provide a comprehensive, high-quality continuum of care to PLWH, and Part C is an essential component of this care continuum. An HIV/AIDS epidemiological analysis, comparing the previous three-year reporting period (1/1/17-12/31/19) to the current reporting period (1/1/18-12/31/20), found that the Sacramento TGA saw increases in AIDS Incidence and Prevalence, HIV Prevalence, and the number of People Living with HIV (PLWH). The largest increase was in AIDS incidence, which increased a staggering 57.5% from 120 to 189 new AIDS cases. This increase in AIDS incidence is disconcerting and highlights the increased need for all PLWH to receive and be engaged in ongoing HIV medical care to achieve viral suppression. AIDS prevalence increased 3.3% (from 2,636 to 2,723), and HIV prevalence increased 7.3% (from 2,445 cases to 2,624 cases). Finally, the number of PLWH in the Sacramento TGA increased 5.2% (from 5,081 to 5,347 PLWH).
