Seattle Indian Health Board, (SIHB), a Federally Qualified Health Center (FQHC) and an Urban Indian Health Program, proposes a five-year project with a budget of $645,594 per year. With this award, SIHB will implement IHS 4-in-1 activities that will address the most urgent health needs in Seattle/King County’s urban American Indian/Alaska Native (AI/AN) community. The purpose of this project is to ensure the highest possible health status for Urban Indians through the delivery of four key programs: 1) health prevention/disease prevention (HP/DP) services; 2) immunization services; 3) alcohol and substance use disorder services; and 4) mental health services. SIHB proposes COPD/asthma as the focus of HP/DP activities, with the goal of identifying individuals with asthma and chronic obstructive pulmonary disease (COPD), stratifying risk level, and improving care coordination for highest risk individuals. With grant funding, we will establish baseline metrics for patients diagnosed with asthma and COPD, implement screening, health education, and integrated care planning, and sustain regular data collection processes and reports to monitor outcomes of care coordination and treatment. Immunization services will protect Seattle/King County AI/ANs from vaccine preventable diseases and integrate vaccine promotion alongside our existing outreach to prevent and vaccinate against COVID-19. Proposed project activities include maintaining immunization activities for children aged birth to five and older vulnerable adults. We will identify clinical vaccination champions for each care team, implement protocols for vaccination outreach and recalls, and implement vaccination quality improvement tracking to make metrics and outcomes visible to the entire clinical team. To address alcohol and substance use disorder (SUD) concerns among Urban AI/ANs, SIHB will sustain access to MAT and provide comprehensive care to help Urban Indians treat OUDs and SUDs. Specifically, we will sustain 2 MAT distribution sites in King County, ensure patients with OUDs receiving MAT also benefit from integrated care including Traditional Indian Medicine and recovery support services, reach an average of 130 unduplicated patients annually with MAT and wrap around services, and scale up new IOP services to reaching individuals with SUDs. Finally, SIHB will address mental health services by implementing universal depression screening for all clients; integrating behavioral health services with medical care and human services, and maintaining a seamless continuum of mental health treatment services to Urban AI/ANs. Integrating mental health services hinges on team-based care with care management and supports for patients to reach health goals and utilize community and personal resources. Care teams will consist of primary care providers, Mental Health Specialists, and Patient Navigator/Schedulers.
